补肾活血法保髋治疗股骨头坏死的疗效观察

摘　　要：目的 探讨中药保髋治疗股骨头坏死的临床疗效。方法 采用回顾性研究方法,收集2018年1月至2022年12月在我院就诊的60例早期(FicatⅠ、Ⅱ期)股骨头坏死患者,依据不同治疗方法分为治疗1组(创伤性股骨头坏死)、治疗2组(非创伤性股骨头坏死)和对照组(包括创伤和非创伤性股骨头坏死),每组20例。三组均有保护性负重的基础治疗,同时,治疗1组和治疗2组以补肾活血中药为基础方,分别采用辨证治疗。疗程2个月,观察三组的髋关节疼痛强度、Harris髋关节评分的改善程度。结果 三组患者性别比例、年龄、病程等基线资料比较,差异无统计学意义(P>0.05)。2个月后随访,治疗1组、2组VAS疼痛评分均降低(P<0.05),Harris评分均提高(P<0.05);治疗1组、2组VAS疼痛评分和Harris评分均较对照组明显改善,差异有统计学意义(P<0.05)。结论 补肾活血中药辨证辨因保髋治疗股骨头坏死具有良好疗效。Objective To explore the clinical efficacy of hip preservation therapy with traditional Chinese medicine in the treatment of osteonecrosis of femoral head.Methods Totally 60 cases with early osteonecrosis of femoral head(Ficat stageⅠandⅡ)treated in our hospital from January 2018 to December 2022 were collected and analyzed retrospectively.They were divided into three groups according to different treatment methods:treatment group 1(traumatic osteonecrosis of femoral head),treatment group 2(non-traumatic osteonecrosis of femoral head)and control group(including traumatic and non-traumatic osteonecrosis of femoral head),with 20 cases in each group.All the three groups had basic treatment of protective weight bearing.At the same time,treatment group 1 and treatment group 2 were treated with syndrome differentiation therapy based on traditional Chinese medicine for tonifying kidney and promoting blood circulation.After a course of 2 months,the intensity of hip pain and the improvement of Harris hip score were observed in the three groups.Results There was no significant difference in gender ratio,age or course of disease among the three groups(P>0.05).After 2 months of follow-up,the VAS scores in treatment group 1 and 2 were decreased(P<0.05),while the Harris scores were increased(P<0.05);the VAS and Harris scores in treatment group 1 and 2 were better than those in control group(P<0.05).Conclusion The application of kidney-tonifying and blood-activating traditional Chinese medicine has a good effect in the treatment of osteonecrosis of the femoral head.
