摘 要:目的研究滞针对肌筋膜疼痛综合征大鼠扳机点形态学的影响。方法取20只健康SD雄性大鼠建立肌筋膜疼痛综合征模型,采用随机数字表法分为模型组和滞针组(每组10只),另取10只健康SD雄性大鼠为对照组,其中对照组不干预;模型组建立模型后不干预;滞针组建立模型后使用滞针每天1次,连续治疗7 d后收集各组大鼠骨骼肌组织,进行HE染色病病理观察及免疫组化检测组织中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)蛋白的表达及定位,通过人工读片进行评分,观察组织病理变化。结果HE染色病理观察:与模型组相比,滞针组肌纤维紊乱程度降低,扭曲、萎缩或断裂明显减少,肌细胞水肿程度降低,核固缩及炎细胞浸润减少。免疫组化实验:与对照组比较,模型组和滞针组骨骼肌纤维IL-6及TNF-α蛋白阳性率均显著升高,差异有统计学意义(P<0.05);与模型组比较,滞针组骨骼肌纤维IL-6及TNF-α蛋白阳性率显著降低,差异有统计学意义(P<0.05)。结论滞针对肌筋膜疼痛综合征大鼠扳机点骨骼肌形态影响较大,可降低骨骼肌纤维中IL-6及TNF-α蛋白阳性表达量,为滞针治疗肌筋膜疼痛综合征的临床研究提供了科学依据。Objective To study the effect of stagnant acupuncture on the morphology of trigger points in rats with myofascial pain syndrome.Methods Twenty healthy male SD rats were selected to establish a model of myofascial pain syndrome.They were randomly divided into a model group and a stagnant needle group(10 in each group)using a random number table method.Another 10 healthy male SD rats were selected as the control group,with no intervention in the control group;The model group does not intervene after establishing the model;After establishing the model in the stagnant needle group,the stagnant needle was used once a day.After continuous treatment for 7 days,skeletal muscle tissue of rats in each group was collected for HE staining pathological observation and immunohistochemical detection of TNF in the tissue-α.The expression and localization of IL-6 protein were evaluated by manual film reading and histopathological changes were observed.Results HE staining pathological observation:Compared with the model group,the needle stagnation group showed a decrease in muscle fiber disorder,distortion,atrophy,or breakage,decreased muscle cell edema,reduced nuclear pyknosis,and reduced inflammatory cell infiltration.Immunohistochemical experiment:Compared with the control group,the skeletal muscle fibers IL-6 and TNF in the model group and needle stagnation group were observed-α.The protein positive rates were significantly increased,with statistically significant differences(P<0.05);Compared with the model group,the skeletal muscle fibers IL-6 and TNF in the stagnant needle group-αThe positive rate of protein significantly decreased,with a statistically significant difference(P<0.05).Conclusion Acupuncture stagnation has a significant impact on the morphology of skeletal muscle at the trigger point in rats with myofascial pain syndrome,which can reduce IL-6 and TNF in skeletal muscle fibers-αThe positive expression level of protein provides a scientific basis for clinical research on the treatment of myofascial pain syndr
