平颤舒筋汤联合盐酸苯海索对帕金森病患者认知功能及多巴胺功能的影响

摘　　要：目的探讨平颤舒筋汤联合盐酸苯海索治疗帕金森病的临床疗效,以及对患者认知功能及多巴胺功能的影响。方法选取医院2021年6月至2022年6月收治的帕金森病患者106例,按信封盲选法分为观察组和对照组,各53例。对照组患者予盐酸苯海索联合普拉克索治疗,观察组患者予盐酸苯海索联合平颤舒筋汤治疗,均治疗3个月。结果观察组治疗总有效率为94.34%,显著高于对照组的79.25%(P<0.05);治疗后,两组患者的中医证候积分、帕金森病综合评分量表(UPDRS)评分、39项帕金森病生存质量量表(PDQ-39)评分均显著降低(P<0.05),蒙特利尔认知评估量表(MoCA)评分均显著升高(P<0.05),且观察组均显著优于对照组(P<0.05);观察组患者脑内多巴胺水平及血清5-羟色胺、神经营养因子-3、脑源性神经营养因子、血管内皮生长因子表达水平均显著高于对照组(P<0.05),胱抑素C、肿瘤坏死因子-α表达水平均显著低于对照组(P<0.05),且观察组均显著优于对照组(P<0.05)。观察组与对照组不良反应发生率相当(11.32%比18.87%,P>0.05)。结论平颤舒筋汤联合盐酸苯海索治疗帕金森病的临床疗效良好,能有效改善患者的临床症状,缓解认知功能障碍,提高脑内多巴胺能神经元功能及神经营养因子水平,抑制炎性反应,且安全性较好。Objective To investigate the clinical efficacy of Pingchan Shujin Decoction combined with benhexol hydrochloride in the treatment of Parkinson's disease,and its effect on cognitive function and dopamine function of patients.Methods A total of 106 patients with Parkinson's disease admitted to the hospital from June 2021 to June 2022 were selected and divided into the observation group and the control group by the envelope blind selection method,with 53 cases in each group.The patients in the control group were treated with benhexol hydrochloride combined with Pramipexole,while the patients in the observation group were treated with benhexol hydrochloride combined with Pingchan Shujin Decoction.Both groups were treated for three months.Results The total effective rate in the observation group was 94.34%,which was significantly higher than 79.25%in the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome score,Unified Parkinson's Disease Rating Scale(UPDRS)score,and Parkinson's Disease Questionnaire-39(PDQ-39)score in the two groups were significantly lower than those before treatment,while the Montreal Cognitive Assessment(MoCA)score in the two groups was significantly higher than that before treatment,and those in the observation group were significantly better than those in the control group(P<0.05);the expression levels of dopamine,serum 5-hydroxytryptamine(5-HT),neurotrophic factor 3(NT-3),brain-derived neurotrophic factor(BDNF),and vascular endothelial growth factor(VEGF)in the observation group were significantly higher than those in the control group(P<0.05),while the expression levels of cystatin C(CysC)and tumor necrosis factor-α(TNF-α)in the observation group were significantly lower than those in the control group(P<0.05),and those in the observation group were significantly better than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was comparable to that in the control group(11.32%vs.18.87%,P>0.05).Conclusion Pingchan Shujin De
