寻常性银屑病马拉色菌感染与相关细胞因子的关系

摘　　要：目的研究寻常性银屑病马拉色菌感染与血清P物质(SP)、肿瘤坏死因子-α(TNF-α)及皮肤人β防御素2(HBD-2)mRNA表达水平的关系。方法回顾性收集2020年1月-2022年6月河南科技大学第二附属医院收治的80例寻常性银屑病马拉色菌感染患者为感染组,随机选取同期医院收治的86例寻常性银屑病未发生马拉色菌感染患者为未感染组,统计两组一般资料及血清SP、TNF-α及皮肤HBD-2 mRNA表达水平;感染组患者根据不同病情严重程度分为轻、中和重度组,分别为28、27和25例,比较不同病情严重程度感染组患者马拉色菌密度,采用Spearman相关性分析马拉色菌密度与寻常性银屑病患者病情严重程度相关性。结果感染组血清SP、TNF-α及皮肤HBD-2 mRNA表达水平高于未感染组(P<0.05)。轻、中、重度组马拉色菌密度为+呈降低趋势,马拉色菌密度为+++呈升高趋势,组间比较,差异具有统计学意义(P<0.05)。Spearman相关性分析显示,马拉色菌密度与寻常性银屑病患者病情严重程度呈正相关关系(r=0.604,P=0.003)。结论马拉色菌感染可引起寻常性银屑病患者外周血SP、TNF-α及皮肤HBD-2 mRNA表达水平变化及病情进展,同时马拉色菌感染与寻常性银屑病患者病情严重程度明显相关。OBJECTIVE To observe the association between Malassezia infection in the patients with psoriasis vulgaris and expression levels of serum substance P(SP),tumor necrosis factor-α(TNF-α)and humanβ-defensin 2(HBD-2)mRNA.METHODS A total of 80 psoriasis vulgaris patients with Malassezia infection who were treated in the Second Affiliated Hospital of Henan University of Science and Technology from Jan 2020 to Jun 2022 were retrospectively collected and assigned as the infection group,meanwhile,86 psoriasis vulgaris patients who did not have Malassezia infection were chosen as the no infection group.The baseline data and the expression levels of serum SP,TNF-αand skin HBD-2 mRNA were statistically analyzed.The patients of the infection group were divided into the mild group with 28 cases,the moderate group with 27 cases and the severe group with 25 cases according to the severity of disease.The density of Malassezia was compared among the patients with different severities of disease in the infection group.Spearman correlation analysis was performed for the association between the density of Malassezia and the severity of illness condition of the patients with psoriasis vulgaris.RESULTS The expression levels of serum SP,TNF-αand skin HBD-2 mRNA were significantly higher than those of the no infection group(P<o.05).The proportion of patients with Malassezia density of+showed a downward trend in the mild,moderate and severe group,while the proportion of patients with Malassezia density of+++showed an upward trend,and there was significant between-group difference(P<0.05).Spearman correlation analysis showed that the density of Malassezia was positively correlated with the severity of illness condition of the patients with psoriasis vulgaris(r=0.604,P=0.003).CONCLUSION The Malassezia infection may result in the changes of expression levels of peripheral blood SP,TNF-αand skin HBD-2 mRNA and progression of illness condition of the patients with psoriasis vulgaris.The Malassezia infection is remarkably associated with t
