肌骨超声诊断痛风性关节炎声像特征分析

摘　　要：目的探讨肌骨超声诊断痛风性关节炎的声像特征。方法选择2020年1月—2022年10月上海市嘉定区中医医院收治的痛风性关节炎患者111例作为观察组;并选择同一时间段内的具有类似症状的其他疾病患者110例作为对照组。两组均采取肌骨超声进行诊断,比较两组超声声像特征。结果两组第一跖趾关节、腕关节、踝关节、膝关节、肩关节受累方面,差异无统计学意义(P>0.05)。超声表现:观察组痛风石、平行线、周围肌腱内高回声发生率高于对照组,差异有统计学意义(P<0.05)。而两组关节腔内高回声点、骨侵蚀、软组织肿胀发生率差异无统计学意义(P>0.05)。血流信号总检出率方面,急性痛风性关节炎高于慢性痛风性关节炎,差异有统计学意义(P<0.05)。结论肌骨超声在痛风性关节炎诊断中应用价值较高,其能够将关节受累情况清楚地显示出来,同时可以反映关节血流情况,为疾病的诊治提供参考。Objective To exploring the ultrasound features of musculoskeletal ultrasound in diagnosis of gouty arthritis.Methods One hundred and eleven patients with gouty arthritis admitted to Traditional Chinese Medicine Hospital of Jiading District in Shanghai from January 2020 to October 2022 were selected as the observation group.And 110 patients with similar symptoms from other diseases within the same time period were selected as the control group.Both groups were diagnosed by musculoskeletal ultrasound,and the ultrasound imaging characteristics of the two groups were compared.Results There were no statistically significant differences in the involvement of the first metatarsophalangeal joint,wrist joint,ankle joint,knee joint,and shoulder joint between the two groups(P>0.05).Ultrasound manifestations:the incidence rates of gout stones,parallel lines,and high echogenicity in the surrounding tendons in the observation group were higher than those in the control group wtih statistically significant differences(P<0.05).There were no statistically significant differences in the incidence rates of high echo points,bone erosion,and soft tissue swelling between the two groups(P>0.05).In the total detection rate of blood flow signals,acute gouty arthritis was higher than chronic gouty arthritis,with a statistically significant difference(P<0.05).Conclusion Musculoskeletal ultrasound has high application value in the diagnosis of gouty arthritis,as it can display joint involvement clearly and reflect joint blood flow to provide reference for the diagnosis and treatment of diseases.
