中国神经外科医生工作强度、工作压力与职业满意度的调查研究

摘　　要：目的了解中国神经外科医生的工作强度、工作压力、职业满意度,为相关政策的制定提供数据支撑.方法采用分层整群随机抽样方法,抽取神经外科医生匿名在线填写问卷.分析其工作时长、工作强度、工作压力、职业满意度,以及职业满意度与各指标的相关性.结果共收回有效问卷2095份.调查对象中,男性为2050人(97.9%);年龄[M(Q_(1),Q_(3))]为39(34,46)岁.调查对象每周总体、临床、科研工作时长的中位数分别为60、50、4 h,每个月手术量的中位数为10台;每日睡眠时长<6 h、未休过年假、认为工作强度高、工作压力大者分别占25.7%(539/2095)、58.5%(1226/2095)、81.1%(1700/2095)、83.4%(1747/2095).对职业满意者占64.2%(1345/2095),其职业满意度与每周工作总时长、工作强度感受、工作压力感受均呈负相关(r值分别为-0.13、-0.20、-0.28,均P<0.05),而与每个月的手术量、每日睡眠时长、休年假时长均呈正相关(r值分别为0.09、0.10、0.08,均P<0.05).结论中国神经外科医生普遍认为工作强度高、工作压力大,职业满意度不容乐观,提示应采取切实可行的措施解决神经外科医生面临的困境.Objective To investigate the work intensity,work stress,and career satisfaction of Chinese neurosurgeons and to provide data support for the formulation of relevant policies.Methods A stratified cluster random sampling method was used to select neurosurgeons to anonymously fill out the questionnaire online.We then analyzed their working hours,work intensity,work pressure,career satisfaction,and the correlation between career satisfaction and various indicators.Results A total of 2095 valid questionnaires were collected.Among the survey respondents,2050(97.9%)were male;the age[M(Q_(1),Q_(3))]was 39(34,46)years old.The median weekly overall,clinical,and scientific research working hours of the survey subjects were 60,50,and 4 hours respectively.The median number of surgeries per month was 1o.The proportions of neurosurgeons with daily sleep time<6 hours,those who had not taken annual leave,and those who believed that their work intensity and pressure were high accounted for 25.7%(539/2095),58.5%(1226/2095),81.1%(1700/2095)and 83.4%(1747/2095)respectively.Those who were satisfied with their careers accounted for 64.2%(1345/2095),and their career satisfaction was negatively correlated with the total hours of work per week,feelings of work intensity,and feelings of work pressure(r values:-0.13,-0.20,and-0.28 respectively,all P<0.05)and positively correlated with the monthly number of surgical operations,daily sleep duration,and annual leave duration(r values:0.09,0.10,and 0.08 respectively,all P<0.05).Conclusion Chinese neurosurgeons generally believe that the work intensity and pressure are high and their career satisfaction is not optimistic,suggesting that practical measures should be taken to solve the challenge faced by Chinese neurosurgeons.
