摘 要:目的探讨肢端肥大症患者生长激素(GH)、胰岛素样生长因子-1(IGF-1)与骨代谢标志物、糖代谢、脂代谢等指标的相关性。方法选取2019年5月至2023年5月该院的37例肢端肥大症患者作为研究对象,分别测定患者血清中GH、IGF-1、25-羟维生素D[25(OH)D]、总Ⅰ型胶原氨基端延长肽(TP1NP)、骨钙素(BGP)、β胶原特殊序列(β-CTX)、钙离子(Ca^(2+))、磷离子、空腹血糖(FBG)、甘油三酯(TG)、胆固醇(CHO)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)水平,分析肢端肥大症患者GH、IGF-1与以上指标的相关性。结果相关性分析结果显示,GH与TP1NP、BGP、β-CTX、磷离子、FBG、TG呈正相关(P<0.05),与HDL呈负相关(P<0.05),与25(OH)D、Ca^(2+)、CHO、LDL、ApoA1、ApoB无关(P>0.05)。IGF-1与TP1NP、BGP、β-CTX、磷离子、TG呈正相关(P<0.05),与HDL呈负相关(P<0.05),与25(OH)D、Ca^(2+)、FBG、CHO、LDL、ApoA1、ApoB无关(P>0.05)。结论GH、IGF-1可能影响肢端肥大症患者的骨代谢、糖脂代谢,可为预防肢端肥大症患者发生骨和骨关节系统并发症、糖脂代谢异常提供数据支持。Objective To investigate the correlation between growth hormone(GH),insulin-like growth factor-1(IGF-1)and bone metabolism markers,glucose metabolism,lipid metabolism and other indicators in acromegaly patients.Methods A total of 37 patients with acromegaly in this hospital from May 2019 to May 2023 were selected as the research objects.Serum GH,IGF-1,25-hydroxyvitamin D[25(OH)D],total typeⅠcollagen amino terminal extension peptide(TP1NP),osteocalcin(BGP),beta isomerized C telopeptide(β-CTX),calcium ion(Ca^(2+)),phosphorus ion,fasting blood glucose(FBG),triglyceride(TG),cholesterol(CHO),high density lipoprotein(HDL),low density lipoprotein(LDL),apolipoprotein A1(ApoA1),apolipoprotein B(ApoB)levels were measured.The correlation between GH,IGF-1 and the above indexes in acromegaly patients was analyzed.Results The results of correlation analysis showed that GH was positively correlated with TP1NP,BGP,β-CTX,phosphorus ion,FBG,TG(P<0.05),and negatively correlated with HDL(P<0.05),but had no correlation with 25(OH)D,Ca^(2+),CHO,LDL,ApoA1,ApoB(P>0.05).IGF-1 was positively correlated with TP1NP,BGP,β-CTX,phosphorus ion and TG(P<0.05),and negatively correlated with HDL(P<0.05).IGF-1 was not correlated with 25(OH)D,Ca^(2+),FBG,CHO,LDL,ApoA1 and ApoB(P>0.05).Conclusion GH and IGF-1 may affect bone metabolism,glucose and lipid metabolism in acromegaly patients,which provides data support for the prevention of bone and joint system complications and abnormal glucose and lipid metabolism in acromegaly patients.
