基于数据挖掘技术探讨针刺治疗中风后复视的选穴规律

摘　　要：目的:采用数据挖掘技术探讨针刺治疗中风后复视的核心腧穴及配伍规律。方法:检索中国期刊全文数据库(CNKI)、中国生物医学文献数据库(Sino Med)、万方数据知识服务平台(Wan Fang)和维普资讯中文期刊服务平台(VIP)建库至2023年1月25日针刺治疗中风后复视的临床研究文献。运用Microsoft Excel 2019软件建立数据库并对腧穴进行描述性统计分析,通过SPSS Modeler 18.0及SPSS Statistics 25.0软件分别进行腧穴关联规则及组间聚类分析。结果:共纳入文献47篇,涉及腧穴57个,总频次435次。高频腧穴为睛明、风池及太阳等;选穴多分布于头面颈项部及下肢部;腧穴归经以足三阳经为主;特定穴多选用交会穴;腧穴关联规则显示“睛明-风池-太阳”为高频穴对;腧穴组间聚类分析得到3个类群。结论:针刺治疗中风后复视以“睛明-风池-太阳”为核心处方,重用阳经,尤以足三阳经为主,遵循远近配穴原则,灵活运用开窍穴,起到醒脑开窍、明目通络之功。Objective:To explore the core acupoints and compatibility of acupoints in treating post-stroke diplopia by using data mining technology.Methods:The clinical research literature on acupuncture treatment for post-stroke diplopia were searched from the databases of CNKI,Sino Med,Wan Fang and VIP from the database establishment till January 25th,2023.Microsoft Excel 2019 software was used to establish a database and make a descriptive statistical analysis of acupoints,and SPSS Modeler 18.0 software and SPSS Statistics 25.0 software were applied to find acupoints association rules and carry out cluster analysis among groups,respectively.Results:A total of 47 articles were included,involving 57 acupoints with a total frequency of 435 times.The high-frequency acupoints included Jingming(BL1),Fengchi(GB20)and Taiyang(EX-HN5).The selected points were mostly distributed on head,face,neck and lower limbs.The meridian tropism of acupoints was mainly the three foot Yang meridians.Confluent points were often selected among specific points.The acupoint sassociation rules showed that BL1-GB20-EX-HN5 was a high-frequency acupoint pair.Three groups were obtained by cluster analysis of acupoints.Conclusion:Acupuncture treatment of post-stroke diplopia takes BL1-GB20-EX-HN5 as the core prescription,attaches great importance to Yang meridians,especially the three foot Yang meridians,follows the principle of matching local points and distal points,and flexibly uses the points for inducing resuscitation,so as to refresh brain,improve eyesight and dredge collaterals.
