石学敏“百会、四神聪深纳久留”治疗卒中相关失眠应用浅析

摘　　要：卒中相关失眠是现代社会常见的脑卒中相关身心障碍疾病,较一般人群而言,临床存在发病率高、治疗不重视与预后不良的现状,迫切需要安全有效的兼顾卒中恢复与睡眠改善的治疗措施。石学敏院士结合古今名家思路和自身多年临床体会,针对卒中相关失眠虚实夹杂、“神不安、阴阳乱”等病机特点,创立了“百会、四神聪深纳久留”针刺法。卒中相关失眠病情重、病位深且病程长。本研究选穴精要,可调神助眠、加强经络联系和扩大治疗范围;针刺手法侧重于针刺效应的实现,可加强针刺感应、促进得气和增长治疗时间。本研究系统化地阐述了“百会、四神聪深纳久留”针刺法在选穴、配伍和手法量学等组方的逻辑和意义,呼应了中医整体观念,经临床研究验证疗效满意,值得在科研与临床工作中进一步推广。Objective:Stroke-related insomnia is a common stroke related physical and mental disorder in modern society,which is of high incidence rate,low attention to treatment and poor recovery.Therefore,it is very urgent to take safe and effective treatment measures to balance stroke recovery and sleep improvement.Academician SHI Xuemin,established the‘DU20 and EX-HN1 deep needling and long retention’acupuncture’according to the pathogenic characteristics of post-stroke sleep disorder,such as the deficiency and excess combination,restless mind and disorder of Yin and Yang,based on the thoughts of ancient and modern famous experts and his own clinical experience.Stroke-related insomnia is characterized by severe illness,deep disease location and long disease course.This treatment method grasps the essence in the selection of acupoints,which has the advantages of regulating Shen and helping sleep,strengthening the connection of meridians,and expanding the scope of treatment.At the same time,the acupuncture manipulation focuses on the realization of acupuncture effect,which has the advantages of strengthening needling response,promoting Qi obtaining,and prolonging treatment time.This study systematically expounds the logic and significance of the acupuncture method of‘DU20 and EX-HN1 deep needling and long retention’in terns of the selection of acupoints,compatibility,manipulation metrology,which echoes the holism concept of TCM,and has been proved effective by clinical researches.The therapy is worthy of further promotion in scientific researches and clinical work in the future.
