摘 要:目的观察骨质疏松专科医师早期进病房介入脆性骨折患者管理对住院期间骨质疏松诊疗及出院后至骨质疏松专科随诊的影响。方法共纳入17713例年龄≥50岁的脊柱和髋部脆性骨折患者,根据在住院期间是否接受骨质疏松专科医师介入管理,分为介入组(8551例)和非介入组(9162例),比较两组脆性骨折患者住院期间接受骨质疏松诊疗及出院后3个月内至骨质疏松门诊随诊情况。结果介入组与非介入组患者相比,接受骨密度检查率(62.99%vs.6.39%)、血清骨代谢生化指标检测率、抗骨质疏松药物治疗率(61.99%vs.48.54%)及出院后3个月内至骨质疏松专科门诊随诊率(14.06%vs.5.63%)均明显升高。结论骨质疏松专科医师早期介入可明显提高脊柱和髋部脆性骨折患者骨密度、骨代谢生化指标的检测率,增加骨质疏松的治疗率,强化骨质疏松骨折患者的管理。Objective To observe the impact of early intervention in the management of patients with fragility fractures by osteoporosis specialists on the diagnosis and treatment of osteoporosis during hospitalization and follow up.Methods A total of 17713 patients older than 50 years with fragility fractures of the spine or hip were included in this study and divided into an interventional group(8551 patients)and a non-interventional group(9162 patients)according to whether they received interventional management by an osteoporosis specialist during their hospitalization.The two groups of patients with fragility fractures were compared in terms of receiving osteoporotic diagnosis and treatment during hospitalization and follow-up within 3 months.Results Patients in the interventional group had a significantly higher rate of detection of bone mineral density(62.99%vs.6.39%),serum bone metabolism biochemical markers,anti-osteoporosis medication(61.99%vs.48.54%),and follow-up within 3 months(14.06%vs.5.63%)compared to the non-interventional group.Conclusion Early intervention by osteoporosis specialists can significantly promote the rate of detection of bone mineral density and biochemical markers of bone metabolism in patients with fragility fractures of the spine or hip,improve the treatment rate of osteoporosis,and increase the follow-up rate of patients.
