基于数据挖掘分析针灸治疗急性期带状疱疹的选穴规律

摘　　要：【目的】运用数据挖掘技术分析针灸治疗急性期带状疱疹(AHZ)的选穴规律。【方法】计算机检索中国知网期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学数据库(CBM)等各大数据库。检索时间为各数据库自建库以来至2023年2月。收集以针灸疗法治疗AHZ的临床研究文献,建立选穴处方数据库,采用SPSS 25.0统计软件和SPSS Modeler 18.0软件进行数据挖掘以分析腧穴配伍规律。【结果】纳入针灸治疗急性期带状疱疹的文献共189篇,共189条针灸处方,涉及116个腧穴,使用频次最高的前5位腧穴为阿是穴、夹脊穴、支沟、后溪、太冲;经脉选用以胆、膀胱、胃经最多;关联度最高的腧穴组合与选用频次对应;通过聚类分析可得到4个聚类群。【结论】针灸治疗AHZ以阿是穴、夹脊穴、支沟、后溪为基础,注重局部取穴与远端取穴相结合,配合辨证论治,以达到清肝泻火、健脾祛湿、行气活血的功效。Objective To analyze the selection rules of acupoint in the treatment of acute herpes zoster(AHZ)using data mining technique.Methods Computer searches were performed on major databases such as CNKI,Wanfang,VIP,and CBM.The search period was from the construction of each database since its inception to February 2023.Clinical research literature on the treatment of AHZ with acupuncture therapy was collected,and a database of selected acupoint prescriptions was established,and SPSS 25.0 statistical software and SPSS Modeler 18.0 software were used to perform data mining in order to analyze the compatibility rules of acupoints.Results A total of 189 articles were included in the literature on acupuncture treatment for AHZ,with 189 acupuncture prescriptions involving 116 acupoints,and the top 5 acupoints with the highest frequency of use were A Shi point,Jiaji(EX-B2),Zhigou(SJ6),Houxi(SI3),Taichong(LR3);the most-frequently seleted meridians were gallbladder,bladder,and stomach meridians;the combinations of the acupoints with the highest degree of correlation corresponded to the frequency of selection;and 4 clustering clusters could be obtained by clustering analysis.Conclusion Acupuncture treatment for AHZ was based on points including A Shi,Jiaji,Zhigou,Houxi,focusing on the combination of local and distal acupoints,together with the diffierentiation of syndrome,in order to achieve the efficacy of clearing liver heat and draining fire,strengthening the spleen and dispelling dampness,and promoting qi movement and activating blood.
