通元针法联合隔药脐灸治疗绝经后女性遗忘型轻度认知功能障碍

摘　　要：【目的】观察通元针法联合隔药脐灸治疗绝经后女性遗忘型轻度认知功能障碍(amnestic mild cognitive impairment,aMCI)患者的临床疗效。【方法】将60例绝经后女性肾虚髓减型aMCI患者随机分为观察组和对照组,每组各30例。观察组给予通元针法联合隔药脐灸治疗,对照组给予普通针刺治疗,隔日治疗1次,连续治疗8周。观察2组患者治疗前后中医证候积分、简易精神状态检查量表(MMSE)及其瞬时记忆部分评分、蒙特利尔认知评估量表(MoCA)及其延迟回忆部分评分、华山版听觉语言学习测验(AVLT-H)及其长延迟回忆评分的变化情况,并评价2组患者的临床疗效。【结果】(1)治疗8周后,观察组的总有效率为86.67%(26/30),对照组为63.33%(19/30),组间比较(秩和检验),观察组的疗效明显优于对照组(P<0.05)。(2)治疗后,2组患者的MMSE及其瞬时记忆部分评分、MoCA及其延迟回忆部分评分、AVLT-H及其长延迟回忆评分均较治疗前明显提高(P<0.01),中医证候积分均较治疗前明显降低(P<0.01);组间比较,观察组对MMSE及其瞬时记忆部分评分、MoCA及其延迟回忆部分评分、AVLT-H及其长延迟回忆评分的提高幅度以及对中医证候积分的降低幅度均明显优于对照组(P<0.05或P<0.01)。【结论】通元针法联合隔药脐灸可有效治疗绝经后女性肾虚髓减型aMCI患者,其疗效优于普通针刺治疗。Objective To observe the clinical efficacy of Tongyuan acupuncture(the therapy for dredging Governor Vessel to regulate mind and conducting qi back to its source)combined with herb-partition umbilical moxibustion in treating amnestic mild cognitive impairment(aMCI)in postmenopausal women with kidney-marrow deficiency type.Methods Sixty postmenopausal women with aMCI of kidney-marrow deficiency type were randomly divided into an observation group and a control group,with 30 patients in each group.The observation group was treated with Tongyuan acupuncture combined with herb-partition umbilical moxibustion,and the control group was treated with conventional acupuncture once every other day for 8 consecutive weeks.Before and after the treatment,the changes of traditional Chinese medicine(TCM)syndrome scores,Mini-Mental State Examination(MMSE)scores and its immediate memory item scores,Montreal Cognitive Assessment Scale(MoCA)scores and its delayed recall item scores,and Auditory Verbal Learning Test-Huashan(AVLT-H)scores and its longdelayed recall item scores in the two groups were observed.After treatment,the clinical efficacy in the two groups was evaluated.Results(1)After 8 weeks of treatment,the total efficacy rate of the observation group was 86.67%(26/30)and that of the control group was 63.33%(19/30),and the intergroup comparison(tested by rank-sum test)showed that the efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)After treatment,the MMSE scores and its immediate memory item scores,MoCA scores and its delayed recall item scores,and AVLT-H scores and its long-delayed recall item scores in the two groups were significantly improved compared with the pre-treatment(P<0.01),and the TCM syndrome scores were significantly reduced compared with the pre-treatment(P<0.01).The intergroup comparison showed that the observation group had stronger efficacy on the increase of MMSE scores and its immediate memory item scores,MoCA scores and its delayed recall item scores
