全麻复合硬膜外麻醉在腹腔镜下结直肠癌根治术患者中的应用

摘　　要：目的:探究全麻复合硬膜外麻醉在腹腔镜下结直肠癌根治术患者中应用效果。方法:选取我院2021年8月至2023年1月收治的72例腹腔镜下结直肠癌根治术患者,将其随机分为对照组和观察组,每组36例。对照组患者术中采用全麻,观察组患者术中采用全麻复合硬膜外麻醉,比较2组患者术中血流动力学指标、术后不同时点疼痛评分以及麻醉不良反应。结果:麻醉前2组患者平均动脉压(MAP)、心率(HR)比较差异无统计学意义(P>0.05),气管插管时、切皮时、术毕观察组患者MAP、HR均优于对照组(P<0.05);术前、术后24 h 2组患者疼痛评分比较差异无统计学意义(P>0.05),术后6 h时观察组患者疼痛评分低于对照组(P<0.05);观察组患者麻醉不良反应发生率低于对照组(P<0.05)。结论:对腹腔镜下结直肠癌根治术患者实施全麻复合硬膜外麻醉,可保持其术中血流动力学稳定,减轻术后早期疼痛,降低麻醉不良反应发生率。Objective To investigate the application effect of general anesthesia combined with epidural anesthesia in patients undergoing laparoscopic radical resection of colorectal cancer.MethodsTotal 72 patients with laparoscopic radical resection of colorectal cancer admitted to our hospital from August 2021 to January 2023 were randomly divided into the control group and the observation group,with 36 patients in each group.The control group received general anesthesia during the operation,while the observation group received general anesthesia combined with epidural anesthesia during the operation.The intraoperative hemodynamic indexes,postoperative pain scores at different time points and the anaesthetic adverse reactions were compared between the two groups.Results There was no significant difference in mean arterial pressure(MAP)and heart rate(HR)between the two groups before anesthesia(P>0.05).The MAP and HR of patients in the observation group were better than those in the control group during endotracheal intubation and skin resection and after operation(P<0.05).There was no significant difference in pain scores between the two groups before and 24 h after operation(P>0.05).The pain score of the observation group was lower than that of the control group at 6 h after operation(P<0.05).The incidence of anaesthetic adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion General anesthesia combined with epidural anesthesia for patients undergoing laparoscopic radical resection of colorectal cancer can maintain the stability of intraoperative hemodynamics,alleviate early postoperative pain,and reduce the incidence of anaesthetic adverse reactions.
