摘 要:目的探讨富血小板血浆(platelet-rich plasma, PRP)在大鼠跟腱损伤及修复中的作用机制, 了解与Ⅰ型胶原蛋白及转化生长因子β1(transforming growth factor β1, TGF-β1)的相关性。方法采用Bedford递增负荷力竭法建立大鼠跟腱损伤模型, 随机数字表法分为3组:对照组、模型组、PRP组, 各20只。PRP组对跟腱损伤大鼠于跟腱伤口处注入100 μl PRP, 每周一次, 其余组在相同时间注射等剂量生理盐水。比较组间Ⅰ型胶原蛋白、TGF-β1、生物力学相关参数、炎症因子差异相关性。结果模型组TGF-β1相对表达水平(2.76±0.56)显著高于对照组(0.87±0.15)和PRP组(1.25±0.14), PRP组显著高于对照组, 差异有统计学意义(P<0.05)。模型组Ⅰ型胶原蛋白表达量(15.23±3.16)显著低于对照组(68.47±8.48)和PRP组(31.52±6.34), PRP组显著高于模型组, 差异有统计学意义(P<0.05)。模型组滑动距离、最大拉力水平显著低于对照组, PRP组显著低于对照组, 差异有统计学意义(P<0.05)。模型组TNF-α、IL-6、C反应蛋白水平显著高于对照组和PRP组, PRP组显著高于对照组, 差异有统计学意义(P<0.05)。结论 PRP及Ⅰ型胶原蛋白可促进大鼠跟腱损伤修复, PRP治疗对急性肌腱损伤修复过程中Ⅰ型胶原蛋白表达具有协同调节作用。Objective To explore the mechanism of platelet rich plasma(PRP)in rat Achilles tendon injury and repair,and understand its relationship with Collagen I and transforming growth factorβ1(TGF-Bi).Methods A rat Achilles tendon injury model was established using the Bedford incremental load exhaustion method.The model was randomly divided into three groups,control group,model group,and PRP group,with 20 rats in each group.In the PRP group,the rats with Achilles tendon injury were injected with 100μl PRP at Achilles tendon wound once a week.The other groups were injected with equal doses of normal saline at the same time.The CollagenⅠ,TGF-β1,biomechanical parameters and inflammatory factors between groups were compared.Results The relative expression level of TCF-β1 in the model group(2.76±0.56)was significantly higher than that in the control group(0.87±0.15)and PRP group(1.25±0.14),and that in the PRP group was significantly higher than that in the control group,with a statistically significant difference(P<0.05).The expression level of CollagenⅠin the model group(15.23±3.16)was significantly lower than that in the control group(68.47±8.48)and PRP group(31.52±6.34),while that in the PRP group was significantly higher than that in the model group,with a statistically significant difference(P<0.05).The sliding distance and maximum tension level of the model group were significantly lower than those of the control group,while those of the PRP group was significantly lower than those of the control group,with a statistically significant difference(P<0.05).The levels of TNF-α,IL-6 and C-reactive protein in the model group were significantly higher than those in the control group and PRP group,and those in the PRP group was significantly higher than those in the control group,with a statistically significant difference(P<0.05).Conclusion PRP and CollagenⅠcan promote the repair of Achilles tendon injury in rats,and PRP treatment has a synergistic regulatory effect on the expression of CollagenⅠduring the
