摘 要:目的比较北京地区变应性鼻炎(AR)与非变应性鼻炎(NAR)日门诊量受环境因素的影响。方法收集2016~2017年因鼻部症状就诊于首都医科大学附属北京同仁医院经血清特异性过敏原检测后鼻炎患者信息,回顾分析环境因素对鼻炎日门诊量分布情况的影响。结果7681例AR与6783例NAR患者的高发年龄均在30~39岁,AR较NAR低龄化,AR以男性多,NAR以女性多,AR秋季就诊率高,NAR春季就诊率高。两者日门诊量均与臭氧(O3)呈正相关,每升高10μg/m^(3)致AR和NAR日门诊量增加2.40%(95%CI:0.94%~3.48%)、3.49%(95%CI:2.03%~4.97%),不同性别、年龄分层的单日效应存在差异。结论AR与NAR日门诊量分布在年龄、性别、时间等存在差异,与O3呈正相关,通过环境因素变化可预测鼻炎日门诊量分布趋势,对鼻炎防治有临床意义。OBJECTIVE To compare the impact of environmental factors on the daily outpatient volume of AR and NAR in Beijing.METHODS Collect information on rhinitis patients who visited Beijing Tongren Hospital from 2016 to 2017 due to nasal symptoms and underwent serum specific allergen testing,and retrospectively analyze the impact of environmental factors on the distribution of daily outpatient volume of rhinitis.RESULTS The high incidence ages of 7681 AR and 6783 NAR patients were 30-39 years old.AR was younger than NAR,with more AR occurring in males and more NAR in females.The incidence of AR in autumn was higher,while NAR in spring was higher.Both daily outpatient volume were positively correlated with O_3.Each increase of 10μg/m^(3) resulted in an increase of 2.40%(95%CI:0.94%-3.48%)and 3.49%(95%CI:2.03%-4.97%)in AR and NAR daily outpatient volume.There were differences in the single day effects of different gender and age stratification.CONCLUSION There are differences in the distribution of daily outpatient volume between AR and NAR in terms of age,gender,and time,and it is positively correlated with O_3.Changes in environmental factors can predict the distribution trend of daily outpatient volume for rhinitis,which has clinical significance for the prevention and treatment of rhinitis.
