摘 要:目的探讨磁共振动态对比增强(DCE)联合T1-三维颅脑容积成像(3D BRAVO)增强技术在泌乳素型垂体微腺瘤诊断中的应用价值。方法回顾性将2021年5月至2022年11月在信阳市中心医院临床确诊的45例高泌乳素血症患者纳入研究,所有患者行垂体常规序列平扫+DCE冠状T1-FSE序列扫描,DCE检查完成后立即注射造影剂行增强冠状T1-3D BRAVO高分辨扫描。将所有患者的MRI图像按照序列组合得到3组图像:A组为常规序列平扫+半剂量DCE。B组为常规序列平扫+冠状位T1-3D BRAVO高分辨扫描。C组包括A组+冠状T1-3D BRAVO高分辨扫描。由2位年资较高、诊断水平较丰富的医师分别对3组图像进行阅片分析,采用McNemar检验比较3组检查对泌乳素分泌型垂体微腺瘤的检出效能。结果A组检测到24个病变(65%),B组检测到16个病变,C组检测到35个病变(85%)。C组对泌乳垂体微腺瘤的检测的敏感度和诊断准确率高于单独的A组或者B组(P<0.01)。结论DCE联合T1-3D BRAVO检查技术可明显提高泌乳素型垂体微腺瘤诊断的敏感度,对于高泌乳素血症,T1-3D BRAVO序列可以作为重要补充,以提高泌乳素型垂体微腺瘤的检出率。Objective To investigate the application value of magnetic resonance DCE combined with T1-3D BRAVO enhancement in the diagnosis of prolactin-secreting pituitary microadenomas.Methods Forty-five patients with clinically diagnosed hyperprolactinemia in Xinyang Central Hospital from May 2021 to November 2022 were retrospectively included in the study.All patients underwent routine pituitary sequence plain scan plus dynamic contrast enhancement(DCE)coronal T1-FSE serial scanning,and enhanced coronal T1-3D BRAVO high-resolution scan was performed after DCE examination.The MRI images of all patients were sequentially combined to obtain three groups of images:Group A was conventional sequential plain scan+half-dose dynamic enhanced scan(DCE).Group B was routine sequential plain scan+coronal T1-3D BRAVO high-resolution scan.Group C consisted of Group A+coronal T1-3D BRAVO high-resolution scans.The images of the three groups were reviewed and analyzed by two physicians with greater seniority and rich diagnostic skills,and the efficacy of the three groups in detecting prolactin-secreting pituitary microadenomas was compared by McNemar test.Results There were 24 lesions(65%)detected in group A,16 lesions detected in group B,and 35 lesions(85%)detected in group C,and group C had higher sensitivity and diagnostic accuracy for the detection of lactating pituitary microadenomas than either group A and group B(P<0.01).Conclusion DCE combined with T1-3D BRAVO technology can significantly improve the sensitivity of diagnosis of prolactin-secreting pituitary microadenomas.For patients with hyperprolactinemia,T1-3D BRAVO sequence can be an important addition to improve the detection of prolactin-secreting pituitary microadenomas.
