心脏超声对高血压病左室肥厚伴左心衰竭患者的诊断价值及心功能分析

摘　　要：目的探讨心脏超声对高血压左心室肥厚伴左心衰竭患者的诊断价值及对心功能的评估作用。方法从2020年1月至2022年12月湖北省钟祥市中医院收治的高血压左心室肥厚病例中选择119例左心衰竭患者与31例无左心衰竭患者,将患者分别设为研究组、对照组。在患者中开展二维心脏超声、实时三维心脏超声检查,对比二维心脏超声、实时三维心脏超声对高血压左心室肥厚患者左心衰竭的诊断结果,对比2组高血压左心室肥厚患者经实时三维心脏超声测量的左心功能相关指标。结果对于高血压左心室肥厚患者发生的左心衰竭进行诊断时,实时三维心脏超声的检出率比二维心脏超声高(P<0.05)。研究组在左心射血分数(LVEF)、舒张早期心室充盈速度峰值与舒张晚期心室充盈速度峰值比值(E/A)两项测得的数值均比对照组低,研究组在左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)测得的数值均比对照组高(P均<0.05)。结论对于高血压左心室肥厚患者发生的左心衰竭,心脏超声可起到良好的诊断作用,尤其是实时三维心脏超声,可提高左心衰竭的检出率,对左心功能进行准确评价,诊断价值良好。Objective To explore the diagnostic value of cardiac ultrasound in patients with hypertensive left ventricular hypertrophy and left heart failure,as well as its role in evaluating cardiac function.Methods One hundred and nineteen patients with left heart failure and 31 patients without left heart failure were selected from the hypertensive left ventricular hypertrophy cases who admitted to Hubei Provincial Zhongxiang Traditional Chinese Medicine hospital from January 2020 to December 2022,and the patients were divided into a study group and a control group,respectively.Two-dimensional echocardiography and real-time three-dimensional echocardiography were conducted in patients,the diagnostic results of two-dimensional echocardiography and real-time three-dimensional echocardiography for left heart failure in patients with hypertensive left ventricular hypertrophy were compared,and the left ventricular function related indicators which measured by real-time three-dimensional echocardiography were compared between two groups of hypertensive left ventricular hypertrophy patients.Results When diagnosing left heart failure in hypertensive patients with left ventricular hypertrophy,the detection rate of real-time three-dimensional echocardiography was higher than that of two-dimensional echocardiography(P<0.05).The values of LVEF and E/A ratio measured in the study group were lower than those of the control group,and the values of LVEDD,LVESD,LVEDV,and LVESV measured in the study group were higher than those of the control group,with P<0.05 for all.Conclusion For patients with hypertensive left ventricular hypertrophy who experience left heart failure,echocardiography can play a good diagnostic role,especially real-time three-dimensional echocardiography,which can improve the detection rate of left heart failure and provide an accurate evaluation of left heart function with good diagnostic value.
