类风湿性关节炎颈椎黄韧带钙化:1例报告及综述

摘　　要：颈椎黄韧带钙化属于二羟焦磷酸钙晶体沉积症(calcium pyrophosphate dihydrate crystal deposition disease,CPPDCDD),最早由日本人发现,女性患病率高,CPPD-CDD是很少发生在脊柱的一类炎性关节病,国内相关报道较少。在过去的很长时间内,类风湿性关节炎与上颈椎的关系成为研究的重点,但临床上不少患者同时患有类风湿性关节炎和颈椎黄韧带钙化,本文报告1例患者,并通过文献回顾,分别论述CPPD沉积和颈椎黄韧带钙化、类风湿性关节炎二者间的关系,总结CPPD的形成机制以及颈椎黄韧带钙化的鉴别诊断,提出类风湿可能会通过CPPD间接加速颈椎黄韧带钙化,从而引起严重的后果。Cervical ligamentum flavum calcification belongs to calcium pyrophosphate dihydrate crystal deposition disease(CPPDCDD),a kind of inflammatory joint disease,and was first discovered in Japanese with female predomination,whereas which is relatively rarely occurs in the spine compared with other sites.The relationship between rheumatoid arthritis and the upper cervical disorder has become the focus of research for long time in the past because many patients suffered from both rheumatoid arthritis and cervical ligamentum flavumcalcification.In this paper we reported one patient,and discusses the relationship between the CPPD cervical ligamentum flavum calcification and rheumatoid arthritis by literature review to summarize the formation mechanism of CPPD and differential diagnosis of the cervical ligamentum flavum calcification,which suggested that rheumatoid may indirectly accelerate cervical ligamentum flavum calcification by CPPD,thus causing serious consequences.
