摘  要:目的:探讨特发性矮小症(ISS)患儿25羟维生素D[25-(OH)D]及类胰岛素样生长因子1(IGF-1)水平,并分析相关影响因素。方法:选取赣州市妇幼保健院2021年7月—2022年10月收治的50例ISS患儿,纳入观察组;另选取同期常规体检的50例健康儿童,纳入对照组;测定并比较两组体格发育指标与血清25-(OH)D、IGF-1水平,分析血清25-(OH)D、IGF-1水平与ISS患儿体格发育指标的相关性,检验血清25-(OH)D、IGF-1水平诊断ISS的价值;记录并对比两组一般资料,经logistic检验ISS发生的影响因素。结果:观察组身高、体重、25-(OH)D、IGF-1水平均较对照组低,差异均有统计学意义(P<0.05);经Pearson相关性分析显示,血清25-(OH)D、IGF-1水平与ISS患儿身高、体重均呈正相关(r>0,P<0.05);绘制受试者操作特征(ROC)曲线发现,血清25-(OH)D、IGF-1水平诊断ISS均有价值;经logistic回归分析显示,家族史、摄入蛋类食品频率<2次/周、摄入肉类食品频率<2次/周、每日户外运动时间<1 h、每日睡眠时间<8 h是ISS发生的影响因素(OR>1,P<0.05)。结论:ISS患儿血清25-(OH)D、IGF-1水平与体格发育有关,可作为ISS诊断辅助指标,且ISS的发生可能受家族史、摄入蛋类与肉类食品频率、每日户外运动时间、每日睡眠时间因素影响。Objective:To investigate the levels of 25 hydroxyvitamin D[25-(OH)D]and insulin-like growth factor-1(IGF-1)in children with idiopathic short stature(ISS),and analyze the relevant influencing factors.Method:A total of 50 children with ISS admitted to Ganzhou Maternal and Child Health Hospital from July 2021 to October 2022 were included in the observation group.In addition,50 healthy children who underwent routine physical examination at the same time were included in the control group.The physical development indicators and serum 25-(OH)D,IGF-1 levels in the two groups were measured and compared,the correlation between serum 25-(OH)D,IGF-1 levels and the physical development indicators of children with ISS was analyzed,and the value of serum 25-(OH)D,IGF-1 levels in the diagnosis of ISS was tested.The general data of the two groups were recorded and compared,and the factors affecting the occurrence of ISS were tested by logistic.Result:The height,weight,25-(OH)D and IGF-1 levels of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum 25-(OH)D and IGF-1 levels were positively correlated with the height and weight of ISS children(r>0,P<0.05).Receiver operator characteristic(ROC)curve showed that serum 25-(OH)D and IGF-1 levels had diagnostic value for ISS.logistic regression analysis showed that family history,frequency of egg intake<2 times/week,frequency of meat intake<2 times/week,daily outdoor exercise time<1 hour and daily sleep time<8 hours were the influencing factors of ISS(OR>1,P<0.05).Conclusion:The levels of serum 25-(OH)D and IGF-1 in children with ISS are related to physical development and can be used as auxiliary indicators for the diagnosis of ISS,the occurrence of ISS may be affected by family history,the frequency of eating eggs and meat,daily outdoor exercise time and daily sleep time.
