参苓白术散合肾气丸加味方联合美沙拉嗪栓对脾肾阳虚型溃疡性结肠炎的疗效观察

摘　　要：目的:探讨参苓白术散合肾气丸加味方与美沙拉嗪栓对脾肾阳虚型溃疡性结肠炎的疗效。方法:选取安徽省中医院肛肠二科在2022年1月—2022年12月间接诊的60例脾肾阳虚型溃疡性结肠炎患者作为研究对象,采用简单随机法将其为2组,对照组采用美沙拉嗪栓塞肛治疗,观察组采用参苓白术散合肾气丸加味方(朝夕互补方)口服配合美沙拉嗪栓塞肛治疗,治疗时间均为8周。观察2组治疗后的临床疗效情况。比较2组治疗后的腹痛、腹泻、腰膝酸软、黏液脓血便、里急后重的中医证候积分情况、Baron肠镜黏膜组织学评分、炎症水平指标。结果:2组治疗后,对照组有效率为80.0%,观察组有效率为93.3%,观察组疗效显著优于对照组,差异有统计学意义;观察组患者除腹痛外,其余中医证候积分与对照组相比显著降低(P<0.05);观察组的Baron肠镜黏膜组织学得分低于对照组(P<0.05);2组患者治疗后白介素-6(IL-6)和肿瘤坏死因子α(TNF-α)明显低于治疗前,白介素-10(IL-10)明显高于治疗前,且治疗后组间和组内差异有统计学意义。结论:在常规治疗的基础上加以参苓白术散合肾气丸加味方能有效地改善临床症状,调节与炎症水平有关的指标,减缓肠道炎症反应。Objective:To explore the efficacy of Shenling Atractylodes Powder combined with modified Shenqi Pills and mesalazine suppository in the treatment of ulcerative colitis due to spleen and kidney yang deficiency.Methods:60 patients with ulcerative colitis of spleen and kidney yang deficiency type who were diagnosed in the Second Department of Anorectal Medicine of Anhui Provincial Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected as the research subjects,and they were divided into 2 groups using simple randomization method,and the control group The treatment group was treated with mesalazine anal embolization,while the observation group was treated with Shenling Baizhu Powder combined with Shenqi Pills Modified Recipe(Zhaoxi Hubu Recipe)orally combined with mesalazine anal embolization.The treatment time was 8 weeks in both cases.Observe the clinical efficacy of the 2 groups after treatment.The TCM syndrome scores of abdominal pain,diarrhea,soreness and weakness of the waist and knees,mucus,pus and blood,tenesmus,Baron colonoscopy mucosal histology scores,and inflammation level indicators were compared between the two groups after treatment.Results:After treatment in the 2 groups,the effective rate in the control group was 80.0%,and the effective rate in the observation group was 93.3%.The efficacy of the observation group was significantly better than that of the control group,and the difference was statistically significant.Except for abdominal pain,the patients in the observation group had other TCM syndrome scores.Compared with the control group,it was significantly lower(P<0.05);the Baron colonoscopy mucosal histology score of the observation group was lower than that of the control group(P<0.05);the levels of interleukin-6(IL-6)and tumor necrosis factor in the 2 groups of patients after treatmentα(TNF-α)was significantly lower than before treatment,interleukin-10(IL-10)was significantly higher than before treatment,and the differences between groups and within groups a
