摘　　要：目的 观察桃红四物汤加减配合冰硝散外敷对踝关节骨折术前消肿的临床疗效。方法 选取2021年7月—2022年7月于山东省中医院创伤骨科住院行手术治疗的踝关节骨折患者60例,采取随机分组法分为对照组和试验组,各30例。2组患者入院后均给予制动、抬高、间断冰敷等治疗。对照组在常规治疗基础上给予七叶皂苷钠静脉滴注,试验组在常规治疗基础上给予桃红四物汤内服联合冰硝散外敷。2组患者均治疗至肿胀消退、满足手术条件时。比较2组患者的肿胀持续时间、消肿速度、张力性水疱发生率以及严重不良事件发生率。结果 治疗后,试验组的肿胀持续时间更短,消肿速度更快(均P<0.05)。2组患者张力性水疱发生率以及不良事件发生率差异无统计学意义(P>0.05)。结论 桃红四物汤加减联合冰硝散外敷治疗踝关节骨折术前的肿胀,疗效确切,可明显缓解骨折早期肿胀的情况,且用药安全,值得进一步推广。Objective To investigate the clinical effect of Taohong Siwu decoction combined with external application of mirabilite and borneol powder in the treatment of preoperative swelling of ankle fractures.Methods From July 2021 to July 2022,60 patients with ankle fractures who underwent surgery in Department of Trauma and Orthopedics of Shandong Provincial Hospital of Traditional Chinese Medicine were selected.Patients were randomly divided into a control group and an experimental group,with 30 people in each group.After admission,both groups of patients were treated with immobilization,elevation,and intermittent ice compresses.The control group was given intravenous infusion of escin sodium in addition to usual treatment.On the basis of conventional treatment,the experimental group was given modified Taohong Siwu decoction combined with external treatment of mirabilite and borneol powder.When the swelling subsided and the conditions for surgery were met,both groups of patients ended the treatment.The duration of swelling,the rate of swelling resolution,the incidence of tension blisters,and the rate of serious adverse events were compared between the two groups.Results After treatment,the effect of the experimental group was better than that of the control group.After treatment,the effect of the experimental group was better than that of the control group.The duration of swelling in the experimental group was shorter and the speed of detumescence was faster(P<0.05).There was no significant difference in the incidence of tension blisters and adverse events between the two groups(P>0.05).Conclusion Modified Taohong Siwu decoction combined with external application of mirabilite and borneol powder is effective in treating the swelling of ankle fracture before operation.It can obviously relieve the early swelling of fracture,and is safe to use,which is worthy of further promotion.
