摘　　要：中医基础理论中,人体肌肉骨骼的生长发育与肾脏密切相关。从肾的生理功能及特点中可以探知:肾藏精,在体合骨,生髓(骨髓及脑髓),说明肾为人体肌肉骨骼生长发育之根本,肾气是影响骨关节炎发生发展的重要因素。骨关节炎在中医学中属“痹证、骨痹”范畴,早期关节肿胀疼痛,活动受限,有骨摩擦音,后期可导致骨赘形成、变形等,严重影响患者生活质量。其病因主要是素体先天禀赋不足,肾气亏虚,精不上荣,外邪侵袭,体内湿气集聚成痰,痰凝血瘀,作用于筋脉关节,导致骨关节疾病。此文从肾藏精主骨生髓理论对骨关节炎的病因病机及治法进行探讨,为骨关节炎的预防和治疗提供进一步的诊治思路。Objective In the basic theory of traditional Chinese medicine,the growth and development of human muscle and bone vessels are closely related to kidney.From the physiological functions and characteristics of the kidney,it can be found that the kidney contains essence,and the main pith(bone marrow and brain pith)is the fundamental for the growth and development of human muscle and bone vessels.These results indicated that kidney qi is an important factor affecting the development of osteoarthritis.Osteoarthritis belongs to the category of“arthralgia”and“bone arthralgia”in traditional Chinese medicine.In the early stage,joints are swollen and painful,the movement is limited,and there is bone friction sound.In the later stage,it can lead to the formation of osteophytes.Deformation and other symptoms,seriously affect the quality of work and life of patients.The main cause lies in the deficiency of natural endowment of the body,deficiency of kidney qi,deficiency of essence,invasion of exogenous pathogenic factors,accumulation of moisture in the body into phlegm,phlegm coagulation and blood stasis,which act on the joints of muscles and veins,leading to the occurrence of this disease.In this paper,the etiology,pathogenesis,differentiation and treatment of osteoarthritis were discussed according to the theory of the kidney contains the essence to dominate the bone and produce the marrow,so as to provide further ideas for the prevention and treatment of osteoarthritis.
