白血病患儿父母疾病进展恐惧 对创伤后应激障碍症状的影响:自悯的中介作用

摘　　要：目的分析自悯在白血病患儿父母疾病进展恐惧(fear of progression,FoP)与创伤后应激障碍(post-traumatic stress disorder,PTSD)症状间的中介作用,为其心理健康管理提供依据。方法便利抽样法选取广州市3所三级甲等医院门诊及住院治疗的219名白血病患儿父母为研究对象,采用父母疾病进展恐惧量表、创伤后应激障碍平民版量表和自悯量表对其进行调查。结果白血病患儿父母FoP总分为(39.56±8.99)分;PTSD症状总分为(34.56±12.03)分;自悯总分为(84.42±10.20)分,其中自我同情及自我批评双因素得分分别为(44.27±6.68)分和(37.87±7.53)分。Spearman分析显示,FoP与PTSD症状呈正相关,自悯与FoP、PTSD症状均呈负相关,自我批评与FoP、PTSD症状均呈正相关(均P<0.01)。自悯在FoP和PTSD症状间起部分中介效应(18.10%),自我批评在FoP和PTSD症状间起部分中介效应(26.97%)。结论FoP既可以直接影响PTSD症状,又可以通过自悯双因素的中介效应间接影响PTSD症状。Objective To investigate the mediating role of self-compassion in the relationship between fear of progression(FoP)and post-traumatic stress disorder and related symptoms(PTSS)in parents of children with leukemia,so as to provide a basis for the psychological health management of these adults.Methods A total of 219 parents of children with leukemia in the Department of Paediatrics in 3 tertiary A hospitals in Guangzhou were the objects through purposive sampling method.Fear of Progression Questionnaire(parent version),the PTSD Checklist(Civilian Version)and self-compassion scale were used for investigation.Results The total score of FoP,PTSS,SCS,CS and RUS in the parents of children with leukemia were(39.56±8.99),(34.56±12.03),(84.42±10.20),(44.27±6.68),and(37.87±7.53)respectively.Spearman correlation analysis showed that FoP positively correlated with PTSS,self-compassion negatively correlated with FoP and PTSS,and RUS positively correlated with FoP and PTSS(all P<0.01).The mediating analysis results showed that self-compassion played a partial mediating role in the relationship between FoP and PTSS(18.10%),and RUS played a partial mediating role in the relationship between FoP and PTSS(26.97%).Conclusions The FoP can not only directly affect PTSS,but also indirectly affect PTSS through the mediating effect of self-compassion.
