老年维持性血液透析患者和照顾者自我调节疲劳对二者生活质量的影响

摘　　要：目的通过主客体互倚模型研究老年维持性血液透析(maintenance hemo dialysis,MHD)患者和照顾者的自我调节疲劳对生活质量的影响。方法便利抽样法选取某院接受治疗的214对老年MHD患者及其主要照顾者为研究对象,采用自我调节疲劳量表和瑞典健康相关生活质量调查表对其进行调查,构建自我调节疲劳对生活质量影响的主客体互倚模型。结果老年MHD患者的自我调节疲劳得分明显高于照顾者(P<0.05),而患者的生活质量得分则明显低于照顾者(P<0.05);患者及照顾者的自我调节疲劳均能负向影响自身的生活质量(P<0.05);老年MHD患者及其照顾者的自我调节疲劳与对方的生活质量呈负相关(P<0.05)。结论老年MHD患者与主要照顾者的生活质量与二者的自我调节疲劳存在交互影响,提示护理人员可以患者与照顾者的二元共同体为中心,探索更加有效的干预措施,提高二者生活质量。Objective To study the effects of self-regulatory fatigue on the quality of life in elderly patients with maintenance hemodialysis(MHD)and their caregivers by using actor-partner interdependence model(APIM).Methods A total of 214 pairs of elderly MHD patients treated in our hospital and their main caregivers were selected by convenient sampling,and investigated by the self-regulatory fatigue scale and Swedish health-related quality of life questionnaire,to construct an APIM of self-regulatory fatigue on quality of life.Results The scores of self-regulatory fatigue of elderly MHD patients were significantly higher than those of caregivers(P<0.05),while the scores of quality of life of patients were significantly lower than those of caregivers(P<0.05);self-regulatory fatigue(negative)of patients and caregivers can affect their own quality of life(P<0.05);self-regulatory fatigue of elderly MHD patients and their caregivers can negatively affect the quality of life of the other party(P<0.05).Conclusions The quality of life of elderly MHD patients and their main caregivers interact with their self-regulatory fatigue,suggesting that nursing staff can explore more effective intervention measures to improve their individual quality of life.
