窄直径种植体在上颌后牙缺失修复中的临床应用

摘　　要：目的:对比评估窄直径种植体应用于小间隙上颌后牙缺失修复,并同期行上颌窦内提升术的临床效果。方法:依照缺牙区近远中牙槽嵴间隙分为:4.1~5.5 mm窄直径种植体组(观察组);>5.5 mm常规种植体组(对照组),两组上颌后牙剩余骨量为2.1~5.6 mm,种植术中同期行上颌窦内提升术,术中未行骨增量技术。两组患者均于术后6个月行上部牙冠修复。不同时期随访观察,经临床和影像学检查,及满意度调查,评价临床成功率。结果:两组种植体并发症及种植体存留率无明显差异。结论:利用窄直径种植体应用于小间隙上颌后牙缺失修复,并同期行上颌窦内提升术,具有手术创伤小、恢复快、患者舒适度好等优点,手术操作简便,易于临床推广应用。Objective:To evaluate the clinical results of narrow diameter implants used for restoration of missing maxillary posterior teeth with a small gap and maxillary sinus lifting.Methods:According to the gap size,the group was divided into 4.1~5.5 mm narrow diameter implant group(observation group)and>5.5 mm conventional implant group(control group),and the residual bone height(RBH)of maxillary posterior teeth in both groups was 2.1~5.6 mm.The two groups underwent simultaneous maxillary sinus lift during implant surgery,and no bone augmentation technique was performed during surgery.The examination items include the degree of adjacent tooth looseness,changes in the sinus cavity of the maxillary sinus,patient satisfaction,bone resorption and complications at the implant edge,and clinical success rate.In both groups,upper crown restorations were performed 6 months after surgery.Results:There was no significant difference between the two groups in terms of implant complications and implant retention rate,in addition to increased patients’comfort and satisfaction,clinical applicability was also improved.Conclusion:Complete preoperative evaluation and analysis,as well as selection of appropriate implants,are clinical success factors in ensuring implant restoration in the posterior maxilla.
