基于灰色关联和结构变动的我国公立医院医疗费用分析

摘　　要：[目的]分析我国公立医院医疗费用情况,为促进公立医院控制费用提供科学依据。[方法]采用灰色关联分析法分析门诊和住院各分项费用的关联程度,采用结构变动度分析法分析门诊和住院费用的结构变动情况。[结果]门诊费用构成中,西药费用占比最多,手术费用和卫生材料费用占比较小。门诊费用关联最大的是卫生材料费用,关联最小的是手术费用。门诊费用中,检查费用、治疗费用和手术费用的结构变动值为正数,呈正向变动;西药费用和中药费用的结构变动贡献率最多,两者贡献率超过了48.10%。住院费用构成中,西药费用占比最多,占比不低于22.45%。住院费用关联最大的是其他费用,关联最小的是西药费用。在住院费用中,检查费用、手术费用和卫生材料费用的结构变动值为正数,呈正向变动;西药费用和中药费用的结构变动值为负数,呈负向变动;西药费用的结构变动贡献率最大。[结论]西药费用和中药费用呈负向变动,且与医疗费用的关联程度较低。治疗费用和卫生材料费用呈正向变动,且与医疗费用的关联程度较高。手术费用在医疗费用中占比较小,且关联程度较低。Objective To analyze the medical expenses of public hospitals and provide a scientific basis for promoting expense control in public hospitals in China.Methods Gray correlation analysis was used to analyze the degree of correlation of outpatient and hospitalization expenses,and degree of structure variation was used to analyze the structural change of outpatient and inpatient expenses.Results In the composition of outpatient expenses,the expenses of western medicine accounted for the largest share,and the expenses of surgery and health materials accounted for a smaller share.The largest correlation of outpatient costs was the cost of health materials,and the smallest correlation was the cost of surgery.In outpatient expenses,the values of structural variation of examination expenses,treatment expenses and surgery expenses were positive and varied in a positive direction.The contribution rates of structural variation of western medicine expenses and Chinese medicine expenses were the most,both of which exceeded 48.10%.In the composition of hospitalization expenses,western medicine expenses accounted for the largest proportion,accounting for no less than 22.45%.The largest correlation of hospitalization costs was other costs,and the smallest correlation was western medicine costs.In hospitalization expenses,the values of structural variation of examination expenses,surgery expenses and health materials were positive and varied in a positive direction.The values of structural variation of western medicine expenses and Chinese medicine expenses were negative,and varied in a negative direction.The contribution rates of structural variation of western medicine expenses were the most.Conclusions The expenses of western medicine and Chinese medicine vary in a negative direction,and have a low correlation degree with medical expenses.The expenses of treatment and health materials vary in a positive direction and have a high correlation degree with medical expenses.The expenses of surgery account for a small proportion of
