茯苓桂枝白术甘草汤治疗肝硬化顽固性腹水的效果及对血清生化指标的影响

摘　　要：目的探讨茯苓桂枝白术甘草汤治疗肝硬化顽固性腹水的效果。方法选取2019年1月至2021年12月本院收治的80例肝硬化顽固性腹水患者为研究对象,根据入院编号的奇偶性将其分为对照组(常规治疗)和观察组(茯苓桂枝白术甘草汤治疗),各40例。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的总胆红素(TBIL)、碱性磷酸酶(ALP)、天冬氨酸转氨酶(AST)、谷氨酰转肽酶(GGT)水平低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)、降钙素原(PCT)水平低于对照组,白细胞介素-10(IL-10)水平高于对照组(P<0.05)。治疗后,观察组的腰围、腹水深度低于对照组,24 h尿量高于对照组(P<0.05)。治疗后,观察组的CD4^(+)、CD4^(+)CD25^(+)、自然杀伤(NK)细胞、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(P<0.05)。结论茯苓桂枝白术甘草汤治疗肝硬化顽固性腹水的效果显著,可有效改善肝功能、免疫及炎症指标,具有重要应用价值。Objective To explore the effect of Fuling Guizhi Baizhu Gancao decoction in the treatment of cirrhosis refractory ascites.Methods A total of 80 patients with cirrhosis refractory ascites admitted in our hospital from January 2019 to December 2021 were selected as the research objects.According to the parity of admission number,the patients were divided into control group(routine treatment)and observation group(treated with Fuling Guizhi Baizhu Gancao decoction),with 40 cases in each group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of total bilirubin(TBIL),alkaline phosphatase(ALP),aspartate transaminase(AST)and glutamyl transpeptidase(GGT)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and procalcitonin(PCT)in the observation group were lower than those in the control group,and the level of interleukin-10(IL-10)was higher than that in the control group(P<0.05).After treatment,the waist circumference,ascites depth of the observation group were lower than those of the control group,and the 24 h urine volume was higher than that of the control group(P<0.05).After treatment,CD4^(+),CD4^(+)CD25^(+),natural killer(NK)cells and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and CD8^(+)was lower than that in the control group(P<0.05).Conclusion Fuling Guizhi Baizhu Gancao decoction has a significant effect in the treatment of cirrhosis refractory ascites,which can effectively improve liver function,immunity and inflammation indicators,and has important application value.
