中药苦参汤湿敷联合中医情志干预对混合痔PPH术后患者创面愈合及疼痛程度的影响

摘　　要：目的探讨中药苦参汤湿敷联合中医情志干预对混合痔吻合器痔上黏膜环形切除术(PPH)术后患者创面愈合及疼痛程度的影响。方法选择2021年1月至2022年4月收治的80例混合痔PPH术后患者为研究对象,根据护理方案将其分为对照组和观察组,各40例。对照组行常规护理干预,观察组在对照组基础上给予中药苦参汤湿敷联合中医情志干预。比较两组的干预效果。结果观察组的甲级愈合率高于对照组,创面愈合时间、肛门坠胀感消失时间短于对照组,差异具有统计学意义(P<0.05)。干预后48 h、7 d,观察组的视觉模拟评分法(VAS)、创面水肿程度评分低于对照组,差异具有统计学意义(P<0.05)。干预7 d后,观察组的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于对照组,差异具有统计学意义(P<0.05)。结论中药苦参汤湿敷联合中医情志干预用于混合痔PPH术后护理中,可改善患者的心理状态,缓解疼痛及创面水肿程度,促进创面愈合,值得推广。Objective To explore the effects of traditional Chinese medicine Kushen decoction wet compress combined with traditional Chinese medicine emotional intervention on wound healing and pain degree of patients with mixed hemorrhoids after procedure for prolapse and hemorrhoids(PPH).Methods A total of 80 patients with mixed hemorrhoids after PPH admitted from January 2021 to April 2022 were selected as the research objects.According to the nursing plan,the patients were divided into control group and observation group,with 40 cases in each group.The control group was given routine nursing intervention,and the observation group was given traditional Chinese medicine Kushen decoction wet compress combined with traditional Chinese medicine emotional intervention on the basis of the control group.The intervention effects of the two groups were compared.Results The grade A healing rate of the observation group was higher than that of the control group,and the wound healing time and the disappearance time of anal bulge were shorter than those of the control group,the differences were statistically significant(P<0.05).At 48 h and 7 d after intervention,the scores of Visual Analogue Score(VAS)and wound edema in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 7 d of intervention,the scores of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine Kushen decoction wet compress combined with traditional Chinese medicine emotional intervention applied in postoperative nursing of mixed hemorrhoids PPH can improve the psychological state of patients,relieve pain and wound edema,and promote wound healing,which is worthy of promotion.
