丙泊酚复合盐酸纳布啡静脉全身麻醉在单侧乳腺肿块切除术中的应用效果

摘　　要：目的观察丙泊酚复合盐酸纳布啡静脉全身麻醉在单侧乳腺肿块切除术中的临床应用效果。方法选取芜湖市第一人民医院2022年1月至12月收治的100例拟择期行单侧乳腺肿物切除术的患者作为研究对象,按照随机数字表法分为N组(50例)、F组(50例),N组采用丙泊酚复合盐酸纳布啡的静脉全身麻醉方法,F组采用丙泊酚复合芬太尼的静脉全身麻醉方法。比较两组患者麻醉时间、手术时间、苏醒时间、丙泊酚用量、术后视觉模拟评分法(VAS)评分、围手术期不良反应发生情况。结果N组患者术后苏醒时间早于F组,麻醉过程中丙泊酚用量少于F组,差异有统计学意义(P<0.05);两组的麻醉时间和手术时间比较,差异无统计学意义(P>0.05)。N组患者术后6 hVAS评分低于F组,差异有统计学意义(P<0.05);两组患者术后12 hVAS评分比较,差异无统计学意义(P>0.05)。两组患者的心动过缓发生率比较,差异无统计学意义(P>0.05);N组患者的恶心呕吐、呼吸抑制、丙泊酚注射痛及呛咳发生率低于F组,差异有统计学意义(P<0.05)。结论单侧乳腺肿块切除术的患者,通过应用丙泊酚复合盐酸纳布啡静脉全身麻醉,可以减轻丙泊酚的注射痛,术后苏醒迅速,术后疼痛程度轻,围手术期不良反应发生率低,值得临床推广。Objective To observe the clinical application effect of Propofol combined with Nalbuphine Hydrochloride general intravenous anesthesia in unilateral breast lumpectomy.Methods A total of 100 patients undergoing elective unilateral mastectomy treated in the First People's Hospital of Wuhu from January to December 2022 were selected as the study objects.They were divided into group N(50 cases)and group F(50 cases)according to random number table method.Group N was given general intravenous anesthesia with propofol combined with nalbuphine hydrochloride,and group F was given general intravenous anesthesia with Propofol combined with Fentanyl.The anesthesia time,surgical time,recovery time,Propafol dosage,postoperative visual analogue scores(VAS),and incidence of perioperative adverse reactions were compared between two groups of patients.Results Patients in group N recovered earlier after surgery than that in group F,and the dosage of propofol were less than that in group F during anesthesia,and the differences were statistically significant(P<0.05).There were no significant differences in anesthesia time and operation time between the two groups(P>0.05).The VAS scores at 6 h after operation in group N was lower than that in group F,and the difference was statistically significant(P<0.05).There was no significant difference in VAS scores between the two groups at 12 h after operation(P>0.05).There was no statistically significant difference in the incidence of bradycardia between the two groups(P>0.05).The incidences of nausea,vomiting,respiratory depression,propofol injection pain and bucking in group N were lower than those in group F,and the differences were statistically significant(P<0.05).Conclusion Patients undergoing unilateral breast lumpectomy are treated by general intravenous anesthesia with Propofol combined with Nabuprofen Hydrochloride can reduce the pain of Propofol injection,wake up quickly after the operation,mild postoperative pain,and have a low incidence of adverse reactions during the perioperat
