摘　　要：目的:探讨社区医务人员上门出诊服务的影响因素。方法:选取2022年10—11月嘉定区13家社区卫生服务中心的200名医务人员作为研究对象,采用自制的电子调查问卷进行调查,采用多因素Logistic回归模型分析社区医务人员上门出诊服务意愿的影响因素。结果:185名医务人员中,上门出诊49名(26.49%),未上门出诊136名(73.51%)。职称、参加住院医师规范化培训、家庭医生、上门出诊补贴、纳入个人绩效考核、上门出诊过程存在的医疗风险程度、上门出诊过程存在的医疗纠纷程度、上门出诊过程存在的人身安全隐患是医务人员上门出诊的影响因素(P<0.05)。有序多因素Logistic回归分析显示,参加过住院医师规范化培训、家庭医生以及上门出诊过程存在的医疗风险程度是医务人员上门出诊的独立影响因素(P<0.05)。结论:参加过住院医师规范化培训、家庭医生以及上门出诊过程存在的医疗风险程度影响医务人员上门出诊意愿,应予以重点关注。Objective:To explore the influencing factors of home visit service of community medical staff.Methods:A total of 200 medical staff from 13 community health service centers in Jiading District from October 2022 to November 2022 were selected as the study subjects.A self-made electronic questionnaire was used for survey,and the influencing factors for the willingness of community medical staff to make home visits were analyzed by using a multi-factor Logistic regression model.Results:Among the 185 medical staff,49 cases(26.49%)made home visits and 136 cases(73.51%)did not make home visits.Professional title,participation in the standardized training of residents,family doctors,home visit subsidy,inclusion of personal performance assessment,the degree of medical risk in the process of home visit,the degree of medical disputes in the process of home visit,and the hidden danger of personal safety in the process of home visit were the influencing factors of home visit of medical staff(P<0.05).Orderly multi-factor Logistic regression analysis showed that,participation in the standardized training of residents,family doctors and the degree of medical risk in the process of home visit were independent influencing factors for home visit of medical staff(P<0.05).Conclusion:The participation in the standardized training of residents,family doctors and the degree of medical risk in the process of home visit may affect the willingness of medical staff to make home visits and should be emphasized.
