摘　　要：胸腹主动脉瘤(TAAA)属于血管外科的急危重症,手术治疗难度大,除了需要对动脉瘤进行处理外,术中还需要对相关内脏动脉进行重建。内脏动脉重建成功与否关系到手术能否成功及患者的预后情况。经过长期的改良,开放手术重建内脏动脉的疗效被逐渐认可,但仍然存在较大的创伤及较高的围手术期并发症发生率、病死率。随着腔内技术的不断发展,一系列新技术、新疗法不断涌现,通过腔内治疗的方法重建内脏分支动脉具有创伤更小、围手术期并发症发生率更低和病死率更低的特点,为TAAA患者的微创治疗带来希望,但需要针对患者的具体情况采取个体化治疗方案。本文对TAAA的腔内治疗进展及腔内重建TAAA内脏分支动脉的方式进行综述。Thoracoabdominal aortic aneurysm(TAAA)is an acute and critical condition in vascular surgery,which is difficult to treat surgically.In addition to the treatment of the aneurysm,it is also necessary to reconstruct the relevant visceral arteries during the surgery,and successfully reconstructed visceral artery is related to the success of the surgery and the prognosis.After long-term improvement,the efficacy of open surgery to reconstruct internal artery is recognized by most scholars,but it is still associated with large trauma and high incidence of perioperative complications and mortality.With the continuous development of endovascular technology and the emergence of a series of new technologies and new therapies,endovascular treatment of visceral branch artery reconstruction has less trauma and incidence of lower perioperative complications and mortality,which brings hope to TAAA patients.However,the individualized treatment for different patients is necessary.This article reviews the progress of endovascular treatment of TAAA and the methods of reconstructing TAAA visceral branch artery.
