摘  要:目的 探讨超极速脉搏波(UFPWV)技术在定量评价高血压患者颈动脉结构及功能变化中的应用价值。方法 收集2021年9月至2022年3月于蚌埠医学院第一附属医院就诊的94例高血压患者的临床资料作为高血压组,并收集同期进行体检的90例健康者作为对照组。分别比较同组受试者左侧与右侧颈总动脉内中膜厚度(IMT)、颈总动脉收缩期起始的脉搏波传导速度(PWV-BS)和颈总动脉收缩期末的脉搏波传导速度(PWV-ES)。比较两组受试者的IMT、PWV-BS、PWV-ES以及不同分级高血压患者的IMT、PWV-BS、PWV-ES。结果 同组受试者双侧颈总动脉的IMT、PWV-BS、PWV-ES比较,差异均无统计学意义(P﹥0.05)。两组受试者的IMT比较,差异无统计学意义(P﹥0.05);高血压组受试者的PWV-BS、PWV-ES均高于对照组,差异均有统计学意义(P﹤0.05)。不同分级高血压患者的IMT、PWV-BS、PWV-ES比较,差异均有统计学意义(P﹤0.05)。Ⅲ级高血压患者的IMT、PWV-BS、PWV-ES均高于Ⅰ级高血压患者,Ⅱ级高血压患者的PWV-ES高于Ⅰ级高血压患者,Ⅲ级高血压患者的PWV-ES高于Ⅱ级高血压患者(P﹤0.05)。结论 UFPWV技术能够在高血压患者颈总动脉结构发生变化之前检测出颈总动脉功能的变化,可作为临床上早期发现高血压患者动脉弹性和功能异常的一种新型、有效的检测方法。Objective To explore the application value of ultrafast pulse wave velocity(UFPWV)technique in evaluating the structural and functional changes of carotid artery in patients with hypertension.Method Clinical data of 94 hypertensive patients admitted in the First Affiliated Hospital of Bengbu Medical College from September 2021 to March 2022 were collected and the patients were enrolled into the hypertension group and 90 healthy subjects during the same period into the control group.The intima-media thickness(IMT)of left and right common carotid artery,the pulse wave velocity at beginning of the systole(PWV-BS)and the pulse wave velocity at ending of the systole(PWV-ES)were compared in the same group of subjects.The IMT,PWV-BS,PWV-ES of the two groups and the IMT,PWV-BS,PWVES of patients with different grades of hypertension were compared.Result There was no statistically significant difference in IMT,PWV-BS and PWV-ES of both common carotid arteries in the same group(P>0.05).There was no statistically significant difference in IMT between the two groups (P>0.05). The PWV-ES and PWV-BS of the hypertension group were higher than those of the control group (P<0.05). There were statistically significant differences in IMT, PWV-BS and PWV-ES among patients with different grades of hypertension (P<0.05). The IMT, PWV-BS and PWV-ES values of the patients with grade III hypertension were higher than those of the patients with grade I hypertension, PWV-ES of patients with grade II hypertension was higher than that of patients with grade I hypertension, and PWV-ES of patients with grade III hypertension was higher than that of patients with grade II hypertension (P<0.05). Conclusion UFPWV technique can detect the functional changes before the structural changes of common carotid artery in hypertensive patients, and can be used as a new and effective detection method for early detection of arterial elasticity and function abnormalities in hypertensive patients.
