针刺治疗对2型糖尿病周围神经病变患者氧化应激水平的研究

摘　　要：目的评价整合针灸疗法对糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)痰瘀互结证患者的临床疗效。方法选择2022年3月—2023年6月在海南省中医院康复科及内分泌科治疗的106个糖尿病周围神经病变患者纳入本次研究,按随机数表法进行分组,对照组和观察组各53例。两组患者均按照现行指南中的治疗方案给予常规治疗,包括控制血糖、营养神经等。观察组在此基础上增加整合针灸疗法,即针刺、艾灸、刺络及穴位埋针等中医特色疗法。观察两组患者治疗前后多伦多临床评分系统积分变化和氧化应激指标的变化。结果治疗后两组患者多伦多临床评分系统积分各项积分均较治疗前明显改善,且观察组的症状评分、感觉检测评分优于对照组,差异有统计学意义(P<0.05)。治疗后两组患者MDA、T-AOC均较治疗前下降,且观察组下降幅度大于对照组,差异有统计学意义(P<0.05)。两组SOD均较治疗前上升,差异有统计学意义(P<0.05)。结论在常规治疗基础上增加整合针灸疗法,可以更加有效地改善DPN痰瘀互结证患者的中医临床症状,其机制可能是通过抑制氧化应激实现的。Objective To evaluate the clinical efficacy of integrated acupuncture therapy on patients with diabetic pe-ripheral neuropathy(DPN)with phlegm and stasis syndrome.Methods 106 patients with diabetes peripheral neuropa-thy who were treated in the rehabilitation department and endocrinology department of Hainan Provincial Hospital of Traditional Chinese Medicine from March 2022 to June 2023 were selected to be included in this study.They were grouped according to the random number table.There were 53 patients in the control group and 53 patients in the ob-servation group.Both groups of patients were given routine treatment according to the current guidelines,including controlling blood sugar and nourishing the nerves.On this basis,the observation group added integrated acupuncture and moxibustion therapy,namely acupuncture,moxibustion,collateral needling,acupoint embedding and other TCM characteristic therapies.Observed the changes in Toronto Clinical Scoring System scores and oxidative stress indica-tors before and after treatment in two groups of patients.Results After treatment,the Toronto Clinical Scoring System scores of both groups of patients improved significantly compared to before treatment,and the symptom scores and sen-sory testing scores of the observation group were better than those of the control group,the difference was statistically significant(P<0.05).After treatment,MDA and T-AOC in both groups of patients decreased compared to before treat-ment,and the observed group showed a greater decrease than the control group,the difference was statistically signifi-cant(P<0.05).The SOD levels in both groups increased compared to before treatment,the difference was statistically significant(P<0.05).Conclusion Adding integrated acupuncture therapy on the basis of conventional treatment can more effectively improve the clinical symptoms of Chinese medicine in patients with DPN phlegm and stasis intercon-nection syndrome,and its mechanism may be realized by suppressing oxidative stress.
