血清胱抑素C、β2微球蛋白、淀粉样蛋白A对早期糖尿病肾病的诊断价值分析

摘　　要：目的分析血清胱抑素C(cystatin C,Cys-C)、β2-微球蛋白(β2-microglobulin,β2-MG)、淀粉样蛋白A(serum amyloid A protein,SAA)对早期糖尿病肾病的诊断价值分析。方法回顾性分析2021年1月—2022年12月期间厦门市第三医院收治的100例糖尿病患者资料,根据诊断结果将其分为早期糖尿病肾病组(48例)与单纯糖尿病组(52例)。两组患者均接受血清Cys-C、β2-MG、SAA检测。观察并比较两组患者血清Cys-C、β2-MG、SAA水平,以及3项指标联合检测与单独检测对于早期糖尿病肾病的诊断灵敏度、特异度与准确度。结果早期糖尿病肾病组血清Cys-C、β2-MG、SAA水平高于单纯糖尿病组,差异有统计学意义(P<0.05)。血清Cys-C、β2-MG、SAA联合检测早期糖尿病肾病阳性46例,单独Cys-C、β2-MG、SAA检测阳性分别为30例、35例、38例。其中3项指标联合检测对于早期糖尿病肾病的诊断灵敏度、特异度与准确度高于单独检测,差异有统计学意义(P<0.05)。结论与单纯糖尿病患者相比,早期糖尿病肾病患者血清Cys-C、β2-MG、SAA表达明显升高,且3项指标联合检测可以提高疾病的诊断效能。Objective To analyze the diagnostic value of serum cystatin C(Cys-C),β2-microglobulin(β2-MG),and serum amyloid A protein(SAA)in early diabetic nephropathy.Methods Data of 100 patients with diabetes admitted to Xiamen Third Hospital from January 2021 to December 2022 were retrospectively analyzed.According to the diagno-sis results,they were divided into early diabetic nephropathy group(48 cases)and simple diabetes group(52 cases).Serum Cys-C,β2-MG and SAA were detected in both groups.The serum levels of Cys-C,β2-MG and SAA,as well as the sensitivity,specificity and accuracy of combined detection and single detection of 3 indicators in the diagnosis of early diabetic nephropathy were observed and compared between the two groups.Results Serum levels of Cys-C,β2-MG and SAA in early diabetic nephropathy group were higher than those in simple diabetic group,and the differ-ence was statistically significant(P<0.05).Serum Cys-C,β2-MG,SAA combined detection of 46 cases of early dia-betic nephropathy positive,Cys-C,β2-MG,SAA positive detection of 30 cases,35 cases,38 cases,respectively.Among them,the sensitivity,specificity and accuracy of the combined detection of 3 indexes were higher than that of single detection for early diabetic nephropathy,and the difference was statistically significant(P<0.05).Conclusion The expressions of Cys-C,β2-MG and SAA in early diabetic nephropathy patients were significantly higher than those in simple diabetic patients,and the combined detection of these three indexes could improve the diagnostic effi-ciency of the disease.
