综合治疗联合糖皮质激素静脉注射治疗对AECOPD合并T2DM患者的效果

摘　　要：目的:探讨综合治疗联合糖皮质激素静脉注射治疗对慢性阻塞性肺疾病急性加重期(AECOPD)合并2型糖尿病(T2DM)患者的效果。方法:选择2019年1月—2022年12月福建省汀州医院收治的65例AECOPD合并T2DM患者作为研究对象。按照随机数表法将患者分为对照组(n=32)和观察组(n=33)。对照组给予综合治疗,观察组在对照组基础上给予糖皮质激素静脉注射治疗,比较两组临床疗效、肺通气功能、糖代谢指标及不良反应发生情况。结果:治疗7 d后,观察组总有效率高于对照组,差异有统计学意义(P<0.05);治疗7 d后,两组第1秒用力呼气容积(FEV_1)、用力肺活量(FVC)、每分钟最大通气量(MV)、潮气量(VT)较治疗前升高,且观察组高于对照组,差异有统计学意义(P<0.05);治疗7 d后,两组空腹血糖(FPG)、餐后2 h血糖(2 h PBG)较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗7 d后,两组糖化血红蛋白(HbA1c)较治疗前略降低,但两组比较差异无统计学意义(P>0.05);两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:静脉注射糖皮质激素联合综合治疗临床效果确切,可明显改善AECOPD合并T2DM患者肺部通气功能,避免血糖恶化,且不增加不良反应发生率。Objective:To explore the effect of comprehensive treatment combined with glucocorticoid intravenous injection on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and type 2 diabetes mellitus(T_(2)DM).Method:A total of 65 patients with AECOPD and T_(2)DM who admitted to Fujian Tingzhou Hospital from January 2019 to December 2022 were selected as the study subjects,the patients were divided into the control group(n=32)and the observation group(n=33)according to the random number table method.The control group was given comprehensive treatment,and the observation group was given intravenous injection of Glucocorticoid on the basis of the control group.The clinical efficacy,pulmonary ventilation function,glucose metabolism index and adverse reactions were compared between two groups.Result:After 7 d of treatment,the total effective rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).After 7 d of treatment,the forced expiratory volume(FEV1),forced vital capacity(FVC),maximum ventilation value(MV)and tidal volume(VT)in the first second of the two groups were higher than those before treatment,and the difference was statistically significant(P<0.05).After 7 d of treatment,the fasting plasma glucose(FPG)and 2-hour postprandial blood glucose(2 h PG)in the two groups decreased compared with those before treatment,and which in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05);after 7 d of treatment,the glycosylated hemoglobin(HbA1c)in the two groups were slightly lower than those before treatment,but there was no significant difference between two groups(P>0.05).There was no significant difference in the incidence of adverse reactions between two groups during treatment(P>0.05).Conclusion:The clinical effect of glucocorticoid intravenous injection combined with comprehensive treatment is definite,which can significantly improve the pulmonary ventilatio
