益肾泄浊汤加减治疗肾虚湿浊证尿毒症的效果

摘　　要：目的:探讨益肾泄浊汤加减治疗肾虚湿浊证尿毒症的效果。方法:选取2020年1月—2022年12月江苏省第二中医院收治的102例肾虚湿浊证尿毒症患者。根据随机数表法将其分为对照组和观察组,各51例。两组均给予常规治疗,观察组给予益肾泄浊汤加减治疗。比较两组临床疗效,治疗前及治疗4周后营养指标、肾功能指标、中医症状积分及不良反应。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗4周后,观察组转铁蛋白(TRF)、血红蛋白(Hb)、前白蛋白(PA)水平均高于对照组,差异有统计学意义(P<0.05)。治疗4周后,观察组血尿素氮(BUN)、血肌酐(Scr)、24 h尿蛋白定量均低于对照组,差异有统计学意义(P<0.05)。治疗4周后,观察组腰膝酸软、食少纳呆、疲倦无力、肢冷畏寒积分均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益肾泄浊汤加减治疗肾虚湿浊证尿毒症可提高治疗效果,改善患者营养指标及肾功能,降低中医症状积分,安全可靠。Objective:To explore the effect of Yishen Xiezhuo Decoction addition and subtraction in the treatment of uremia with kidney deficiency and dampness turbidity syndrome.Method:A total of 102 cases of uremia with kidney deficiency and dampness turbidity syndrome admitted to the Jiangsu Second Traditional Chinese Medicine Hospital from January 2020 to December 2022 were selected.They were divided into control group and observation group according to random number table method,51 cases in each group.Both groups were given conventional treatment,and the observation group was given Yishen Xiezhuo Decoction addition and subtraction treatment.The clinical efficacy,nutritional indexes,renal function indexes,traditional Chinese medicine symptom scores before treatment and after 4 weeks of treatment and adverse reactions were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 4 weeks of treatment,the levels of transferrin(TRF),hemoglobin(Hb)and prealbumin(PA)in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).After 4 weeks of treatment,blood urea nitrogen(BUN),blood creatinine(Scr)and 24 h urinary protein quantitation in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).After 4 weeks of treatment,the scores of waist and knee weakness,eating less and staying in a daze,fatigue and weakness,and limb cold fear in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Yishen Xiezhuo Decoction addition and subtraction can improve the therapeutic effect in the treatment of uremia with kidney deficiency and dampness turbidity syndrome,improve the nutritional index and renal funct
