司库奇尤单抗注射液联合阿维A对斑块型银屑病的临床观察

摘　　要：目的:探究司库奇尤单抗注射液联合阿维A治疗斑块型银屑病的临床效果。方法:选取2021年10月—2022年12月江苏大学附属医院收治的74例斑块型银屑病患者作为研究对象,根据随机数表法将患者分为观察组37例与常规组37例。常规组给予阿维A治疗,观察组在常规组基础上联合司库奇尤单抗注射液治疗。比较两组临床疗效、银屑病面积和严重程度指数(PASI)评分、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、免疫功能(CD3^(+)、CD4^(+)/CD8^(+))及不良反应发生情况。结果:两组PASI评分组间、时间及交互比较,差异有统计学意义(P<0.05);治疗4周后、治疗12周后,两组PASI评分均低于治疗前,且观察组低于常规组,差异有统计学意义(P<0.05)。观察组总有效率高于常规组,差异有统计学意义(P<0.05)。治疗后,两组IL-6、TNF-α低于治疗前,且观察组低于常规组,差异有统计学意义(P<0.05)。治疗后,两组CD3^(+)、CD4^(+)/CD8^(+)高于治疗前,且观察组高于常规组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:司库奇尤单抗注射液联合阿维A治疗斑块型银屑病,能有效降低患者的PASI评分及炎症因子水平,改善免疫功能,临床疗效优越且安全可靠。Objective:To explore the clinical effect of Secukinumab Injection combined with Acitretin in the treatment of plaque psoriasis.Method:A total of 74 patients with plaque psoriasis who admitted to Affiliated Hospital of Jiangsu University from October 2021 to December 2022 were selected as the study objects,they were randomly divided into the observation group of 37 cases and the conventional group of 37 cases according to the random number table method.The conventional group was treated with Acitretin,while the observation group was treated with Secukinumab Injection on the basis of the conventional group.The clinical efficacy,psoriasis lesion area and severity index(PASI)score,inflammatory factors[interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)],immune function(CD3^(+),CD4^(+)/CD8^(+))and adverse reactions of two groups were compared.Result:There were significant differences in PASI scores between two groups in terms of groups,time and interaction(P<0.05);after 4 weeks and 12 weeks of treatment,the PASI scores of two groups were lower than those before treatment,and those in the observation group were lower than those in the conventional group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the conventional group,and the difference was statistically significant(P<0.05).After treatment,the levels of IL-6 and TNF-αin two groups were lower than those before treatment,and those in the observation group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After treatment,the levels of CD3^(+)and CD4^(+)/CD8^(+)in two groups were higher than those before treatment,and those in the observation group were higher than those in the conventional group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:Secukinumab Injection Combined with Acitretin in the treatment of plaque
