地诺前列酮和宫颈扩张球囊在足月产妇促子宫颈成熟与引产中的效果及安全性  

Effect and Safety of Dinoprostone and Cervical Dilatation Balloon in Promoting Cervical Maturation and Inducing Labor in Full-term Parturient

摘　　要：目的:探讨地诺前列酮和宫颈扩张球囊在足月产妇促子宫颈成熟与引产中的效果及安全性。方法:选择2019年4月—2021年8月广州中医药大学第一附属医院白云医院收治的135例足月产妇为研究对象,按照随机数表法分为两组,均接受促子宫颈成熟与引产,药物组(n=67)给予地诺前列酮,球囊组(n=68)给予宫颈扩张球囊。观察两组子宫颈成熟效果、分娩情况、新生儿结局及并发症发生率。结果:球囊组子宫颈成熟效果优于药物组,差异有统计学意义(P<0.05)。球囊组引产到分娩时间、总产程短于药物组,产后出血量少于药物组,剖宫产率及子宫颈裂伤发生率低于药物组,差异有统计学意义(P<0.05)。球囊组新生儿结局优于药物组,差异有统计学意义(P<0.05)。球囊组并发症总发生率为11.76%,低于药物组的29.85%,差异有统计学意义(P<0.05)。结论:相比较于地诺前列酮进行引产和促子宫颈成熟,宫颈扩张球囊可更好地促进子宫颈成熟,改善分娩情况及新生儿结局,且产后并发症发生率更低。Objective:To explore the efficacy and safety of Dinoprostone and cervical dilatation balloon in promoting cervical maturation and inducing labor in full-term parturient.Method:A total of 135 full-term parturients admitted to Baiyun Hospital of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from April 2019 to August 2021 were selected as the study objects.They were divided into two groups according to random number table method.All of them received cervical maturation promotion and labor induction;the drug group(n=67)received Dinoprostone,the balloon group(n=68)received cervical dilatation balloon.Cervical maturation effect,delivery situation,neonatal outcome and complication rate were observed in the two groups.Result:The maturation ripening effect of balloon group was better than that of drug group,and the difference was statistically significant(P<0.05).The time from labor induction to delivery and total labor course in balloon group were shorter than those in drug group,the postpartum blood loss was less than that in drug group,and the incidence of cesarean section and cervical laceration were lower than those in drug group,the differences were statistically significant(P<0.05).The neonatal outcome of balloon group was better than that of drug group,and the difference was statistically significant(P<0.05).The total incidence of complications in balloon group was 11.76%,which was lower than 29.85%in drug group,and the difference was statistically significant(P<0.05).Conclusion:Compared with Dinoprostone for inducing labor and promoting cervical maturation,cervical dilatation balloon can better promote cervical maturation,improve delivery and neonatal outcomes,and reduce the incidence of postpartum complications.
