摘　　要：目的:探究钬激光联合一次性取石网篮治疗输尿管上段结石的效果。方法:选择2019年5月—2021年5月深圳市宝安区石岩人民医院、深圳市龙华区人民医院接诊的124例输尿管上段结石患者开展研究,按照随机抽签法分为对照组(予以钬激光治疗)、观察组(予以钬激光联合一次性取石网篮治疗),各62例。比较两组的临床疗效、手术相关情况、结石清除效果、术后并发症发生率及随访12个月期间的复发情况。结果:两组手术时间、术中出血量、术后住院时间比较,差异无统计学意义(P>0.05)。观察组一次性碎石成功率、结石清除率高于对照组,结石逃逸率明显低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);随访12个月,观察组复发率为1.67%(1/60),低于对照组的18.75%(9/48),差异有统计学意义(χ^(2)=7.341,P=0.007)。结论:钬激光联合一次性取石网篮的结石清除效果较理想,且安全可靠,复发率低。Objective:To explore the effect of holmium laser combined with disposable lithotomy net basket in the treatment of upper ureteral calculi.Method:A total of 124 patients with upper ureteral calculi admitted to Shiyan People's Hospital of Baoan District of Shenzhen,and Longhua District People's Hospital of Shenzhen from May 2019 to May 2021 were selected for the study.According to random drawing method,they were divided into control group(holmium laser treatment)and observation group(holmium laser combined with disposable lithotomy net basket treatment),with 62 cases in each group.The clinical efficacy,operation related situation,stone removal effect,postoperative complication rate and recurrence during 12 months of follow-up were compared between the two groups.Result:There were no significant differences in operation time,intraoperative blood loss and postoperative hospital stay between the two groups(P>0.05).The success rate of one-time lithotripsis and stone clearance rate in observation group were higher than those in control group,and the stone escape rate was significantly lower than that in control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).After 12 months of follow-up,the recurrence rate in the observation group was 1.67%(1/60),which was lower than 18.75%(9/48)in the control group,the difference was statistically significant(X^(2)=7.341,P=0.007).Conclusion:The effect of holmium laser combined with disposable lithotomy net basket is ideal,safe and reliable,and the recurrence rate is low.
