弹力线结扎在混合痔外剥内扎术中的应用效果

摘　　要：目的:探讨弹力线结扎在混合痔外剥内扎术中的应用效果。方法:选取2022年6—12月南京中医药大学附属南京中医院收治的70例混合痔患者作为研究对象,按随机数表法将其分为观察组(35例)和对照组(35例)。观察组采用弹力线结扎,对照组采用丝线结扎。比较两组疼痛程度、肛缘水肿评分、创面出血评分、创面愈合时间、结扎痔核数、脱线时间、线圈孔径、肛门功能、住院时间及住院费用。结果:观察组术后3 d、7 d疼痛评分低于对照组,差异有统计学意义(P<0.05);观察组术后7 d创面出血评分低于对照组,差异有统计学意义(P<0.05),观察组最后1根脱线时间及线圈孔径均优于对照组,差异有统计学意义(P<0.05),观察组创面愈合时间早于对照组,差异有统计学意义(P<0.05)。两组手术前后肛门功能比较,差异无统计学意义(P>0.05)。两组住院时间、住院费用、肛缘水肿评分、结扎痔核数比较,差异无统计学意义(P>0.05)。结论:使用弹力线结扎痔核可减轻术后疼痛及出血情况,脱线时间短,创面愈合更快。Objective:To explore the application effect of ligation with elastic thread in external stripping and internal ligation of mixed hemorrhoids.Method:Seventy patients with mixed hemorrhoids admitted to Nanjing Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from June to December 2022 were selected as the study objects,and they were divided into observation group(35 cases)and control group(35 cases)according to random number table method.The observation group was ligation with elastic thread,and the control group was ligation with silk thread.The pain degree,anal edge edema score,wound bleeding score,wound healing time,number of hemorrhoids ligation,detachment time,coil aperture,anal function,length of hospital stay and hospitalization cost were compared between the two groups.Result:The pain scores of the observation group at 3 d and 7 d after surgery were lower than those of the control group,the differences were statistically significant(P<0.05).The wound bleeding score at 7 d after surgery in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The last string-off time and coil aperture in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).The wound healing time in the observation group was earlier than that in the control group,the difference was statistically significant(P<0.05).There were no significant differences in anal function between the two groups before and after surgery(P>0.05).There were no significant differences in hospital stay,hospital cost,anal edema score and number of hemorrhoids ligation between the two groups(P>0.05).Conclusion:The use of ligation with elastic thread of hemorrhoids can reduce postoperative pain and bleeding,and the time of detachment is short,and the wound healing is faster.
