子宫颈双球囊联合缩宫素引产时球囊不同放置时间对引产效果与母婴结局的影响

摘　　要：目的:探讨子宫颈双球囊联合缩宫素引产时球囊不同放置时间对引产效果与母婴结局的影响。方法:将2016年6月—2020年6月福建省福能集团总医院收治的150例足月产妇作为研究对象,按照球囊放置时间将其分为对照组(n=75)和观察组(n=75)。对照组子宫颈球囊放置时间为12 h,观察组子宫颈球囊放置时间为10 h,比较两组子宫颈成熟效果、引产效果、产妇不良结局(宫缩过强、产后出血、产褥感染)及新生儿不良结局(宫内感染、胎儿窘迫、新生儿窒息)。结果:两组总有效率、球囊取出后临产时间、产程比较,差异无统计学意义(P>0.05);观察组产妇不良结局发生率(4.41%)低于对照组(15.87%),差异有统计学意义(P<0.05);两组新生儿不良结局比较,差异无统计学意义(P>0.05)。结论:子宫颈双球囊联合缩宫素引产时球囊放置10 h促子宫颈成熟效果、引产效果与球囊放置12 h相当,能改善产妇结局,且不会增加新生儿不良结局。Objective:To study the effect of different placement time of cervical double balloon combined with Oxytocin on labor induction effect and maternal and infant outcomes.Method:A total of 150 full-term parturients who admitted to Fujian Funeng Group General Hospital from June 2016 to June 2020 were selected as the study objects,and the parturients were divided into the control group(n=75)and the observation group(n=75)according to balloon placement time.The control group was placed for 12 h,and the observation group was placed for 10 h.The effects of cervical ripening,labor induction effect,maternal adverse outcomes(excessive uterine contraction,postpartum hemorrhage,puerperal infection)and neonatal adverse outcomes(intrauterine infection,fetal distress,neonatal asphyxia)were compared between two groups.Result:There were no significant differences in the total effective,labor time after balloon removal and labor process between two groups(P>0.05).The incidence of adverse outcomes in the observation group(4.41%)was lower than 15.87%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in neonatal adverse outcomes between two groups(P>0.05).Conclusion:The effect of balloon placement for 10 h on cervical ripening and induction effect of labor in cervical double balloon combined with Oxytocin induction of labor is equivalent to that of balloon placement for 12 h,it can improve maternal outcomes and will not increase neonatal adverse outcomes.
