优秀运动员肌腱末端病临床诊疗及效果分析

摘　　要：目的:研究聚焦式冲击波联合超声电导疗法在运动员肌腱末端病的临床应用价值。方法:对南京体育学院运动康复医院门诊收治的64例现役运动员肌腱末端病患者采用非手术治疗,随机分为观察组和对照组各32例,观察组采用聚焦式冲击波联合超声电导治疗,对照组采用针灸、按摩和超声波等传统物理治疗,疗程4周,观察聚焦式冲击波联合超声电导治疗与传统疗法之间的效果有无差异。结果:经相同疗程治疗后:①观察组和对照组的VAS评分均较治疗前显著降低,观察组VAS评分低于对照组(P<0.05);②观察组治疗后恢复专项训练的总有效率(显效+有效)为90.6%,高于对照组的总有效率71.9%(P<0.05),差异具有统计学意义。所有运动员在治疗过程中均未出现任何副作用及明显不良反应。结论:采用聚焦式冲击波联合超声电导疗法治疗运动员肌腱末端病可使疼痛快速且明显缓解,提高了专项训练的时间和效率,疗效显著。因此,推荐使用该疗法用于运动员肌腱末端病的治疗。OBJECTIVE:This study aims to investigate the clinical application value of combining focused shockwave therapy with ultrasound-guided treatment in elite athletes suffering from tendon insertion pain.METHODS:We enrolled 64 actively competing athletes with tendon insertion pain treated at the Outpatient Rehabilitation Hospital of Nanjing Sport Institute.All participants received non-surgical treatments and were randomly divided into two groups,an observation group and a control group,each consisting of 32 cases.The observation group received focused shockwave therapy in combination with ultrasound guidance,while the control group underwent traditional physical treatments,including acupuncture,massage,and ultrasound.The treatment period spanned 4 weeks,with a focus on assessing the potential differences in effectiveness between focused shockwave therapy with ultrasound guidance and traditional treatment methods.RESULTS:After an equivalent treatment duration,(1)both the observation and control groups exhibited a significant reduction in VAS scores compared to before treatment,with the VAS scores in the observation group being lower than those in the control group(P<0.05).(2)The observation group achieved a total effective rate(significant improvement+improvement)of 90.6%for the recovery of specific training,surpassing the control group's total effective rate of 71.9%(P<0.05),with the difference being statistically significant.No adverse effects or notable complications were observed in any of the athletes during the treatment process.CONCLUSION:The use of focused shockwave therapy in conjunction with ultrasound guidance for the treatment of tendon insertion pain in elite athletes results in rapid and significant pain relief,enhances the efficiency and duration of specific training,and produces remarkable therapeutic outcomes.Consequently,this therapy is recommended for the treatment of tendon insertion pain in athletes.
