摘  要:目的观察新疆农牧区老年肌肉减少症(后简称肌少症)患者外周血单个核细胞(PBMC)中肿瘤坏死因子样细胞凋亡弱诱导物(TWEAK)、成纤维细胞生长因子诱导早期反应蛋白14(Fn14)及核因子κB(NF-κB)通路基因的表达变化,并探讨其意义。方法在新疆农牧区常住居民中选择80例研究对象,肌少症患者40例纳入肌少症组、健康人群40例纳入对照组。提取两组空腹外周血中的PBMC,采用qRT-PCR法检测TWEAK、Fn14、IκB激酶α(IKKα)、IκB激酶β(IKKβ)、NF-κB p65 mRNA。结果肌少症组外周血PBMC中TWEAK、Fn14、IKKα、IKKβmRNA表达高于对照组(P均<0.05)。校正性别、年龄后,协方差分析结果仍显示,肌少症组外周血PBMC中TWEAK、Fn14、IKKα、IKKβmRNA表达高于对照组(P均<0.05)。校正年龄、性别后,偏相关性分析显示,TWEAK mRNA与Fn14 mRNA表达呈正相关(r=0.424,P<0.01),Fn14 mRNA与IKKα、IKKβmRNA表达呈正相关(r分别为0.264、0.284,P均<0.05),IKKα、IKKβmRNA与NF-κB p65 mRNA表达呈正相关(r分别为0.724、0.660,P均<0.01)。结论新疆农牧区老年肌少症患者外周血PBMC中TWEAK、Fn14、IKKα、IKKβmRNA高表达,TWEAK、Fn14 mRNA表达与IKKα、IKKβmRNA表达有相关性;TWEAK、Fn14可能通过调节IKK激活NF-κB炎症通路从而参与老年人肌少症的发生发展。Objective To observe the expression of tumor necrosis factor-like weak inducer of apoptosis(TWEAK),fibroblast growth factor-induced early reactive protein 14(Fn14)and nuclear factor-κB(NF-κB)in peripheral blood mononuclear cells(PBMC)of patients with senile sarcopenia(later referred to as sarcopenia)in agricultural and pastoral areas of Xinjiang,and to explore their significance.Methods Eighty participants were selected from the permanent residents in agricultural and pastoral areas of Xinjiang.Forty patients with sarcopenia were included in the sarcopenia group and 40 healthy people were included in the control group.PBMC was extracted from fasting peripheral blood of the two groups,and TWEAK,Fn14,inhibitor of IκB kinase-α(IKKα),inhibitor of IκB kinase-β(IKKβ)and NF-κB p65 mRNA were detected by qRT-PCR.Results The mRNA expression levels of TWEAK,Fn14,IKKαand IKKβin PBMC of the sarcopenia group were higher than those of the control group(all P<005).After adjusting for sex and age,the results of covariance analysis still showed that the mRNA expression levels of TWEAK,Fn14,IKKαand IKKβin PBMC of the sarcopenia group were higher than those of the control group(all P<0.05).After adjusting for age and sex,partial correlation analysis showed that the expression of TWEAK mRNA was positively correlated with the expression of Fn14 mRNA(r=0.424,P<0.01),the expression of Fn14 mRNA was positively correlated with the mRNA expression levels of IKKα,IKKβ(r=0.264 and 0.284,respectively;both P<0.05),and the mRNA levels expression of IKKα,IKKβwere positively correlated with the expression of NF-κB p65 mRNA(r=0.724 and 0.660,respectively;both P<0.01).Conclusions TWEAK,Fn14,IKKαand IKKβmRNA are highly expressed in PBMC of patients with senile sarcopenia in agricultural and pastoral areas of Xinjiang.The expression of TWEAK and Fn14 mRNA is correlated with the expression of IKKαand IKKβmRNA.TWEAK and Fn14 may participate in the development and progression of senile sarcopenia by regulating IKK to activate NF-κ
