摘  要:目的探讨不同程度支气管肺发育不良(BPD)患儿血清白细胞介素-33(IL-33)、基质金属蛋白酶-16(MMP-16)、维生素D结合蛋白(VDBP)水平变化及临床意义。方法选取2019年6月至2021年12月保定市第二中心医院早产128例BPD患儿作为研究组,另选取同期同院无BPD早产儿64例作为对照组,比较两组血清IL-33、MMP-16、VDBP水平;根据吸入氧浓度(FiO_(2))将研究组分为轻度、中度、重度BPD亚组,对比不同程度亚组血清IL-33、MMP-16、VDBP水平,Spearman相关分析各血清指标水平与病情程度的相关性,受试者工作特征(ROC)曲线及曲线下面积(AUC)评估预测价值。结果研究组血清IL-33、MMP-16水平高于对照组,VDBP水平低于对照组,差异有统计学意义(t值分别为16.291、12.451、12.067,P<0.05);随病情程度加重血清IL-33、MMP-16水平增高,VDBP水平降低(F值分别为169.273、108.204、65.075,P<0.05);Spearman相关分析显示血清IL-33、MMP-16水平与病情程度呈正相关,VDBP水平与病情程度呈负相关(r值分别为0.855、0.779、-0.833,P<0.05);血清IL-33、MMP-16、VDBP联合鉴别轻度与中度BPD的AUC为0.900(0.821~0.952),联合评估重度BPD的AUC为0.941(0.885~0.975),均大于各指标单一评估;不同程度亚组治疗前、治疗4周后血清IL-33、MMP-16水平均与疗效呈负相关,VDBP水平均与疗效呈正相关(P<0.05)。结论BPD患儿血清IL-33、MMP-16、VDBP水平与病情程度、疗效密切相关,可作为临床评估的重要辅助指标。Objective To investigate changes in serum interleukin-33(IL-33),matrix metalloproteinase-16(MMP-16),vitamin D binding protein(VDBP)levels in children with different degrees of bronchopulmonary dysplasia(BPD)and their clinical significance.Methods One hundred and twenty-eight preterm infants with BPD in Baoding Municipal Second Central Hospital from June 2019 to December 2021 were selected as study group,and another 64 preterm infants without BPD in the same period were selected as control group.Serum IL-33,MMP-16 and VDBP levels of the preterm infants were compared between the two groups.The preterm infants in the study group were divided into three subgroups as mild BPD subgroup,moderate BPD subgroup,and severe BPD subgroup according to inspired oxygen fraction(FiO_(2)),and the serum levels of IL-33,MMP-16 and VDBP of the preterm infants were compared among different subgroups.Spearman correlation analysis was conducted to assess correlations between levels of various serum indicators and severity of the condition.Receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to evaluate predictive values of them.Results The serum IL-33 and MMP-16 levels of the preterm infants in the study group were higher than those in the control group,while their VDBP level was lower than that in the control group(t=16.291,12.451 and 12.067 respectively,all P<0.05).As the severity of the disease worsened,the serum levels of IL-33 and MMP-16 increased,while VDBP level decreased(F=169.273,108.204 and 65.075 respectively,all P<0.05).Spearman correlation analysis showed that the serum levels of IL-33 and MMP-16 were positively correlated with the severity of the disease,while the serum level of VDBP was negatively correlated with the severity of the disease(r=0.855,0.779 and-0.833 respectively,all P<0.05).The AUC of the combination of serum IL-33,MMP-16 and VDBP for distinguishing mild BPD from moderate BPD was 0.900(0.821-0.952),while the AUC of the combination for distinguishing mild BPD and moderate BPD fr
