摘 要:目的探讨低对比剂浓度、低管电压方案CTU在非结石性上尿路梗阻诊断中的应用价值。方法选取经CT平扫及超声检查提示上尿路梗阻且梗阻原因排除结石性上尿路梗阻患者52例,所有患者均在我院行双源CT低浓度、低管电压CTU且CT平扫与CTU检查时间间隔7天以内,对获得的图像在Syngo.via后处理工作站进行图像重建及分析,将分析结果与病理结果或临床最终诊断结果进行对比分析,评价双低剂量CTU在非结石性上尿路诊断有效性。结果52例上尿路梗阻原因均得到明确诊断,其中19例为先天发育异常所致,17例为内源性肿瘤所致,6例为外源性肿瘤压迫所致,10例为炎性病变所致。结论CT双低剂量CTU所获得图像质量能够满足非结石性上尿路梗阻诊断需求,并可以显著降低对比剂剂量及辐射剂量。Objective To explore the application value of dual-source CTU with low contrast agent concentration and low tube voltage in the diagnosis of non-calculous upper urinary tract obstruction.Methods 52 patients with upper urinary tract obstruction were diagnosed by CT plain scan and ultrasonography,and the causes of obstruction were excluded.All patients underwent dual-source CT with low concentration and low tube voltage CTU in our hospital,and the time interval between CT plain scan and CTU examination was not more than 7 days.The obtained images were reconstructed and analyzed in Syngo.via post-processing workstation.The analysis results were compared with pathological results or clinical final diagnosis structure to evaluate the effectiveness of double low-dose CTU in the diagnosis of non-calculous upper urinary tract.Results The causes of upper urinary tract obstruction in 52 cases were clearly diagnosed,of which 19 cases were caused by congenital dysplasia,17 cases were caused by endogenous tumors,6 cases were caused by exogenous tumor compression,and 10 cases were caused by inflammatory lesions.Conclusion The image quality obtained by dual-source CT dual low-dose urography can meet the diagnostic requirements of non-calculous urinary tract obstruction,which can significantly reduce the contrast agent dose and radiation dose.
