摘  要:目的比较精子制备过程中添加肌醇(Myo-inositol,MYO)和D-手性肌醇(D-chiro-inositol,DCI)对弱精子症患者精液常规参数和生物功能参数的影响。方法将2021年4月至2022年11月在攀枝花市妇幼保健院就诊的40例弱精子症不育男性患者液化后的精液标本分为三等份,37°C培养箱中孵育30 min后,采用非连续密度梯度离心法进行精子制备。对照组:不进行干预;MYO组:孵育前在精液中和梯度离心培养液中均加入2 mg/mL MYO;DCI组:孵育前在精液中和梯度离心培养液中均加入750 ug/mL DCI。分析制备后的精液常规参数,使用流式细胞仪分别测定3组精液标本的生物功能参数,包括精子DNA碎片指数(DNA fragmentation index,DFI)、活性氧水平(reactive oxygen species,ROS)、线粒体膜电位(mtochondrial membrane potential,MMP)。结果使用非连续密度梯度离心法进行精子制备后:(1)MYO组显著提高制备后前向运动精子百分率和精子回收率(P<0.05);(2)与对照组相比,MYO显著降低精子DFI(P<0.05),而DCI组与对照组DFI差异无统计学意义(P>0.05),MYO组和DCI组对精子的ROS水平和MMP水平均无显著影响(P>0.05)。结论在体外添加MYO与精子共孵育能提高精子活力,MYO是精子制备过程中降低精子DNA氧化损伤的有效抗氧化保护剂。Objective To compare the effects of Myo-inositol(MYO)and D-chiral-inositol(DCI)on semen routine parameters and biological function parameters in patients with asthenospermia.Methods The semen samples of 40 infertile men with asthenospermia in Panzhihua Maternal and Child Health Hospital from April 2021 to November 2022 were divided into three equal parts.After30 min incubation in a 37C incubator,sperm was prepared by discontinuous density gradient centrifugation.Control group:no intervention;MYO group:2 mg/mLof MYO was added to the semen and gradient centrifugation medium before incubation;DCI group:750 ug/mL DCI was added to both the semen and gradient centrifugation medium before incubation.The routine parameters of semen after preparation were analyzed,and the biological function parameters of three groups of semen samples were measured by flow cytometry,including sperm DNA fragment index(DFI),reactive oxygen species level(ROS)and mitochondrial membrane potential(MMP).Results Afer sperm preparation by discontinuous density gradient centifugation:MYO group significantly increased the percentage of forward motile sperm after preparation and improved the sperm recovery rates of sperm preparation technique(P<0.05).②Compared with the control group,MYO significantly decreased sperm DFI(P<0.05),while there was no statistically significant difference in DFI betwern the DCI group and the control group(P>0.05).MYO group and DCI group had no significant effect on sperm ROS and MMP levels(P>0.05).Conclusion Adding MYO to co-incubate with sperm in vitro can improve sperm motility,and MYO is an effective antioxidant protective agent that reduces oxidative damage to sperm DNA during sperm preparation.
