奥瑞姆自理模式联合舒适护理对行假体隆乳术患者术后并发症发生率的影响

摘　　要：目的探究奥瑞姆自理模式联合舒适护理在行假体隆乳术患者中的实施效果。方法回顾性研究2018年1月至2022年12月于陆军军医大学第一附属医院行假体隆乳术的94例患者的一般资料。将2018年1月至2020年12月收治的47例患者纳入对照组,并对其实施常规护理;将2021年1月至2022年12月收治的47例患者纳入观察组,并对其实施奥瑞姆自理模式联合舒适护理。比较2组的术后并发症发生率、恢复情况、疼痛程度及舒适度。结果与对照组相比,观察组的术后并发症总发生率更低(P<0.05)。观察组术后首次下床和引流管拔除的时间均早于对照组,住院时间较对照组更短,术后24 h睡眠时间较对照组更长(P均<0.05)。术后1 d、3 d和5 d,2组的视觉模拟评分法评分均较术后当日降低(P均<0.05),且观察组均较同期对照组更低(P均<0.05)。2组在出院时和出院1个月后的舒适状况量表各维度评分均比术前高(P均<0.05),且观察组均较同期对照组更高(P均<0.05)。结论奥瑞姆自理模式联合舒适护理可改善行假体隆乳术患者的恢复情况,减轻其疼痛程度,提升其舒适度,且能减少术后并发症的发生。Objective To evaluate the implementation effect of Orem's self-care model combined with comfort nursing in patients undergoing prosthetic breast augmentation.Methods A retrospective study was conducted on the general data of 94 patients who underwent prosthetic breast augmentation at the First Affiliated Hospital of the Army Medical University between January 2018 and December 2022.The 47 patients admitted between January 2018 and December 2020 were included in the control group and treated with routine nursing.The 47 patients admitted between January 2021 and December 2022 were included in the observation group and treated with Orem's self-care model and comfort nursing.The incidence of postoperative complications,recovery,pain level,and comfort level were compared between the two groups.Results The overall incidence of postoperative complications was lower in the observation group than in the control group(P<0.05).The observation group had shorter time to first postoperative ambulation,shorter time to drainage tube removal,shorter hospitalization time,and longer sleep time within 24 h after surgery compared with the control group(all P<0.05).On the 1st,3rd,and 5th days after surgery,the Visual Analogue Scale scores of both groups decreased compared with those on the day after surgery(all P<0.05),and the observation group had lower scores than the control group(all P<0.05).At discharge and 1 month after discharge,the scores of all dimensions of the General Comfort Questionnaire in both groups increased compared with those before surgery(all P<0.05),and the observation group had higher scores than the control group(all P<0.05).Conclusion The Orem's self-care model combined with comfort nursing can improve the recovery,relieve the pain,enhance the comfort,and reduce the postoperative complications in patients undergoing prosthetic breast augmentation.
