益气活血方对急性心肌梗死大鼠交感神经重构的影响

摘　　要：目的:观察益气活血方对急性心肌梗死大鼠心脏急性损伤期交感神经重构的影响,探究其作用机制。方法:通过结扎大鼠心脏左冠状动脉前降支血管,构建SD大鼠急性心肌梗死模型,造模前随机选取20只大鼠仅穿线不结扎作为假手术组,造模成功大鼠随机分组为模型组、益气活血方组、美托洛尔组各20只。术后24 h益气活血方组、美托洛尔组大鼠分别灌胃相应药物,模型组大鼠灌胃蒸馏水。给药3、7 d检测心脏功能、室颤易感性、心肌组织病理形态、酪氨酸羟化酶(TH)阳性交感神经纤维分布情况、血浆神经肽Y(NPY)含量,以及TH、生长相关蛋白43(GAP43)、神经生长因子(NGF)、信号素3A(Sema-3A)蛋白表达。结果:给药3、7 d,模型组大鼠左心室射血分数(LVEF)、左心室短轴率(LVFS)均低于假手术组(P<0.01),左心室舒张末期内径(LVIDd)、左心室收缩末期内径(LVIDs)均高于假手术组(P<0.01)。给药3 d,益气活血方组大鼠LVIDs低于模型组(P<0.05);给药7 d,益气活血方组和美托洛尔组大鼠LVEF、LVFS均高于模型组(P<0.05或P<0.01),LVIDs均低于模型组(P<0.05);美托洛尔组大鼠给药7 d后的LVIDs低于给药3 d(P<0.05)。给药3、7 d,模型组大鼠电刺激诱发室颤阈值低于假手术组(P<0.01或P<0.05);给药7 d,美托洛尔组大鼠电刺激诱发室颤阈值高于模型组(P<0.01);美托洛尔组大鼠给药7 d后电刺激诱发室颤阈值高于给药3 d(P<0.05)。给药3、7 d,模型组大鼠心肌细胞断裂且排列紊乱,细胞核固缩,大量炎症细胞浸润。给药3 d,益气活血方组和美托洛尔组大鼠心肌细胞结构紊乱,部分坏死。给药7 d,益气活血方组较模型组改善明显;美托洛尔组心肌细胞排列有明显整齐改变。给药3、7 d,模型组大鼠TH阳性神经纤维密度明显增加,分布紊乱。给药3 d,益气活血方组、美托洛尔组大鼠TH阳性神经纤维密度低于模型组;给药7 d,益气活血方组、美托洛尔组大Objective:To observe the effect of Yiqi Huoxue formula on sympathetic nerve remodeling in rats with acute myocardial infarction during acute cardiac injury and explore its mechanism.Methods:The acute myocardial infarction model of SD rats was established by ligating the anterior descending branch of the left coronary artery in rats.Before modeling,20 rats were randomly selected as the sham operation group without ligation.The successful rats were randomly divided into model group,Yiqi Huoxue formula group and metoprolol group,with 20 rats in each group.Rats in Yiqi Huoxue formula group and metoprolol group were respectively intragastrically administered with corresponding drugs,and rats in model group were intragastrically administered with distilled water after 24 hours of modeling.Cardiac function,susceptibility to ventricular fibrillation,pathological morphology of myocardial tissue,distribution of tyrosine hydroxylase(TH)-positive sympathetic nerve fibers,plasma neuropeptide Y(NPY)content and protein expression of TH,growth associated protein 43(GAP43),nerve growth factor(NGF)and Sema-3A were detected after 3 and 7 days of administration.Results:On the 3rd and 7th day after administration,the model group showed lower LVEF and LVFS than sham operation group(P<0.01),while higher LVIDd and LVIDs than sham operation group(P<0.01).On the 3rd day after administration,Yiqi Huoxue formula group showed lower LVIDs than model group(P<0.05).On the 7th day after administration,the Yiqi Huoxue formula group and metoprolol group showed higher LVEF and LVFS than model group(P<0.05 or P<0.01),while lower LVIDs than model group(P<0.05).The LVIDs of metoprolol group after 7 days of administration was lower than those after 3 days of administration(P<0.05).On the 3rd and 7th day after administration,the model group showed lower threshold of ventricular fibrillation induced by electrical stimulation,than sham operation group(P<0.01 or P<0.05).On the 7th day after administration,the metoprolol group showed higher threshold of ven
