PRP并髋关节外科脱位打压植骨支撑术治疗早期股骨头坏死效果

摘　　要：目的观察富血小板血浆(PRP)联合髋关节脱位打压植骨支撑术治疗早期股骨头坏死的近期临床效果和安全性。方法选取股骨头坏死病人60例(60髋),随机分为对照组和观察组,各30例。对照组予以髋关节外科脱位植骨支撑术,观察组在对照组的基础上联合PRP注射。术后随访12~18月,平均(14.50±2.36)月,观察并比较两组手术前后髋关节功能评分(Harris评分)、视觉模拟评分法(VAS)评分和引流量、术后并发症。结果两组病人手术切口均一期愈合,无感染和静脉血栓形成等并发症。术后12个月两组Harris评分、VAS评分均较术前改善,且观察组Harris评分及VAS评分均优于对照组,差异有统计学意义(t=5.625、3.702,P<0.05)。观察组引流量低于对照组,差异有显著性(t=-4.60,P<0.05)。结论PRP联合髋关节外科脱位植骨支撑术治疗早期股骨头坏死近期疗效确切,且无严重并发症,值得临床上进一步推广应用。Objective To observe the short-term clinical efficacy and safety of platelet-rich plasma(PRP)plus surgical hip dislocation combined with impacting bone grafts in the treatment of early-stage osteonecrosis of the femoral head(ONFH).Methods A total of 60 patients with ONFH(60 hips)were selected and divided into observation group and control group accor-ding to the random number table method,with 30 patients in each group.Patients in the observation group received PRP injection plus surgical hip dislocation combined with impacting bone grafts,while those in the control group received surgical hip dislocation combined with impacting bone grafts.The postoperative follow-up period was 12-18 months(mean:(14.50±2.36)months).Harris hip scores,visual analogue scale(VAS)scores,postoperative drainage volume,and postoperative complications were compared between the two groups.Results All surgical incisions in the two groups healed by first intention without complications such as infection and venous thrombosis.At 12 months postoperatively,Harris hip score and VAS score were improved in both groups,and were significantly better in the observation group than in the control group(t=5.625,3.702;P<0.05).Moreover,the drainage volume in the observation group was significantly less than that in the control group(t=-4.60,P<0.05).Conclusion PRP plus surgical hip dislocation combined with impacting bone grafts has good short-term efficacy in the treatment of early-stage ONFH,without serious complications.This method is worthy of further clinical application.
