摘  要:目的探讨全身运动评估在高危脑损伤新生儿脑发育随访中的作用。方法选取2020年1月—2022年10月我院收治的102例高危脑损伤病儿,采用全身运动质量评估(GMs)量表评估病儿在扭动运动阶段和不安运动阶段的全身运动质量,随访病儿至12月龄,分析GMs量表对高危脑损伤病儿脑发育的诊断效能。结果102例高危脑损伤新生儿经GMs量表评估,扭动运动阶段正常70例(68.63%),单调性26例(26.49%),痉挛同步性6例(5.88%);不安运动存在67例(65.69%),不安运动缺乏35例(34.31%)。GMs量表对新生儿脑发育异常的诊断准确度为78.43%,灵敏度为85.71%,特异度为77.89%,阳性预测值为22.22%,阴性预测值为98.67%。随访至12月龄时,GMs量表与Gesell发育量表评估结果的一致性较差(Kappa=0.202,P>0.05)。结论GMs量表对高危脑损伤新生儿早期脑发育情况的预测准确度、灵敏度和阴性预测值均较高,可为后续及时采取有效的干预提供指导,长期随访需要结合全面发育商检查等手段。Objective To investigate the role of whole body exercise assessment in the follow-up of brain development in neonates with high-risk brain injury.Methods A total of 102 children with high-risk brain injury who were admitted to our hospital from January 2020 to October 2022 were enrolled,and General Movements Assessment(GMs)scale was used to evaluate the whole body exercise quality of children during the stages of writhing movements and fidgety movements.The children were followed up to 12 months of age to analyze the diagnostic efficacy of GMs scale for brain development in children with high-risk brain injury.Results The GMs scale assessment showed that among the 102 neonates with high-risk brain injury,there were 70 neonates with normal movements(68.63%),26 with monotonic movements(26.49%),and 6 with spasmodic synchronous movements(5.88%)in the stage of writhing movements,and there were 67 neonates with fidgety movements(65.69%)and 34 neonates with a lack of fidgety movements(34.31%).The GMs scale has an accuracy of 78.43%,a sensitivity of 85.71%,a specificity of 77.89%,a positive predictive value of 22.22%,and a negative predictive value of 98.67%in the diagnosis of abnormal brain development in neonates.At 12 months of age,there was poor consistency between the assessment results of GMs scale and those of Gesell Developmental Scale(Kappa=0.202,P>0.05).Conclusion The GMs scale has high accuracy,sensitivity,and negative predictive value in predicting the early brain development of neonates with high-risk brain injury and can provide guidance for timely and effective intervention in the future.The methods including comprehensive developmental quotient testing should be used for long-term follow-up.
